Summary:
Responsible for providing clinical nutrition services, including nutrition assessment, nutrition diagnosis and intervention to manage patient care. Coordinates nutritional aspects of patient's care with physicians and health care personnel. Utilizes the Nutrition Care process to document in the medical record. Resource professional for medical staff/students. Serves on committees representing Food and Nutrition Services. Ensures patient satisfaction, quality care, regulatory agency compliance, and good public relations are achieved through the safe and efficient use of resources. . Perform other related duties as required. Responsible for maintaining necessary credentials for the position.
Responsibilities:
Utilize information systems (daily rounds report, dept. mail, nutrition processor) to prioritize list of patients in need of nutrition services. Assesses nutrition status of pediatric, adolescent, adult and geriatric patients through documentation in the medical record within time frame indicated. Uses anthropometrics, nutrition focused physical findings, diet history; interpret lab values, medication and other medical information. Identifies appropriate nutrition diagnosis with proper documentation/format. Develops documents and revises a nutrition care plan based on information from the patient, medical record, family and health care team members. Consults with other disciplines as appropriate. Addresses and documents discharge plan. Collaborates with home care/discharge planners or social workers on nutrition support needs for discharge. Plan and prescribe appropriate nutrition therapy within scope of expertise and privileging for desired patient outcome. Identify and initiate appropriate nutrition interventions: therapeutic diet, oral nutrition supplements, enteral support (formula type/rate, addition of modular), parenteral support, nutrition related labs. Teach and counsel patients and families on appropriate nutrition therapy for disease states or prevention and wellness, desired outcomes and behavior objectives. Evaluates effectiveness/outcomes of intervention/therapies and modifies treatment/interventions when necessary.
Credentials:
Registered Dietitian (Required)

Salary Range:
$55,607.14 - $88,682.88/Annually based on 40 hours per week As part of their job offer, successful candidates are provided a specific rate, taking into consideration various factors including experience and education.
