$45-$50 per hour Position Summary Care Connection Rehabilitation Center is seeking a compassionate, knowledgeable, and experienced Registered Dietitian to join our interdisciplinary care team. In this essential role, you will oversee the nutritional care of our residents, develop individualized dietary plans, ensure regulatory compliance, and promote overall health and wellness throughout our facility. The ideal candidate will have prior skilled nursing facility experience, strong clinical nutrition knowledge, and a passion for providing resident-centered care. Key Responsibilities Assess residents' nutritional status, dietary needs, preferences, and nutritional risks. Develop, implement, and monitor individualized nutrition care plans. Review physician orders and ensure compliance with therapeutic diets and nutritional interventions. Participate in interdisciplinary care plan meetings and resident conferences. Coordinate dietary services with nursing, social services, activities, rehabilitation, and environmental services departments. Utilize the Resident Assessment Instrument (MDS) to develop and evaluate nutrition care plans. Maintain accurate documentation, assessments, progress notes, and dietary records. Provide nutrition education and counseling to residents, families, and staff. Ensure substitute meals of equal nutritional value are provided when residents decline scheduled menu selections. Monitor food service operations to ensure compliance with sanitation, safety, and quality standards. Assist with state and federal surveys and implement corrective action plans as needed. Review food service cost reports and assist with budgetary and cost-control initiatives. Maintain compliance with HIPAA regulations and resident rights standards. Participate in departmental meetings, quality assurance activities, and facility committees. Qualifications Current, active, and unrestricted Registered Dietitian credential in New Jersey. Bachelor's Degree in Dietetics, Nutrition, or a related field from an accredited institution. Minimum of two (2) years of experience in a Skilled Nursing Facility (SNF) or long-term care setting preferred. Prior nursing home or SNF experience required. Strong knowledge of clinical nutrition, therapeutic diets, food service management, and long-term care regulations. Experience with SigmaCare and GeriMenu preferred. Ability to analyze food service costs, budgets, and financial reports. Excellent communication, leadership, organizational, and interpersonal skills. Strong understanding of federal and New Jersey dietary regulations within long-term care settings.
