Job Description
The Registered Dietitian is responsible for planning, coordinating, and managing nutrition services to meet the clinical, regulatory, and dietary needs of residents in the skilled nursing setting. The RD ensures residents receive individualized, evidence-based nutrition care while supporting quality outcomes, survey readiness, and overall facility wellness initiatives. Essential Duties & Responsibilities Clinical Nutrition Management Complete comprehensive nutrition assessments, including initial, quarterly, annual, significant change, and PRN evaluations. Develop and revise individualized care plans based on resident needs, diagnoses, preferences, and risk factors. Monitor nutritional status, intake, weight trends, lab values, skin integrity, and hydration. Provide diet education for residents, families, and staff as needed. Make recommendations for modified diets, supplements, and interventions to support optimal nutrition and clinical outcomes. Participate in wound care rounds to ensure appropriate nutritional support. Regulatory & Documentation Compliance Document assessments, care plans, progress notes, and interventions in accordance with federal (CMS), state of Nevada regulations, and facility policies. Ensure menus and diet orders comply with nutritional standards and physician directives. Assist with preparation and participation in state surveys; respond to surveyor inquiries regarding nutrition services. Interdisciplinary Collaboration Collaborate with nursing, therapy, dietary, social services, and medical providers to support comprehensive resident care. Attend and actively participate in IDT meetings, care plan conferences, QAPI initiatives, and weight/skin meetings. Menu & Food Service Oversight Review and approve menus for nutritional adequacy and regulatory compliance. Work with the Dietary Manager and kitchen staff to ensure proper meal service, portion control, therapeutic diets, and food safety standards. Provide training and education to dietary staff on nutrition topics, special diets, and best practices. Quality Assurance & Performance Improvement Conduct monthly audits such as tray accuracy, weight monitoring, meal service, sanitation, and satisfaction surveys. Identify trends and implement corrective action plans to support high-quality nutrition services and resident outcomes. Participate in QAPI projects related to nutrition, weight management, and overall well-being. Qualifications Registered Dietitian (RD/RDN) with active Commission on Dietetic Registration (CDR) credential. Must hold a current Nevada state license or be eligible for licensure. Prior experience in a SNF, LTC, or clinical healthcare setting preferred. Knowledge of CMS regulations, Nevada health codes, and MDS/Care Plan processes beneficial. Strong communication, time management, and organizational skills. Ability to work independently and collaboratively within an interdisciplinary team.
