Job Description

Overview Join our dedicated home health team as a compassionate and skilled Dietitian, where you will play a key role in improving the nutritional health and well-being of patients in the comfort of their own homes. In this rewarding position, you will assess patients' nutritional needs, develop individualized nutrition care plans, and provide education and support to patients and caregivers. Your expertise will help patients manage chronic conditions, promote healing, prevent complications, and maintain independence while aging in place. This position offers the opportunity to make a meaningful impact through personalized, patient-centered care in a collaborative home health environment. Duties Conduct comprehensive nutrition assessments during home visits to evaluate nutritional status, dietary habits, medical history, and environmental factors affecting nutrition. Develop and implement individualized nutrition care plans that align with physician orders, medical conditions, patient goals, and cultural preferences. Provide medical nutrition therapy for patients with chronic diseases such as diabetes, heart failure, kidney disease, COPD, and other nutrition-related conditions. Assess and manage specialized nutrition needs, including enteral nutrition (tube feeding), wound healing support, weight management, and therapeutic diets. Educate patients and caregivers on meal planning, food preparation, disease management, medication-nutrient interactions, and healthy lifestyle practices. Collaborate with physicians, nurses, therapists, social workers, and caregivers to coordinate comprehensive patient care. Monitor patient progress through ongoing evaluations and adjust nutrition interventions as needed to achieve desired outcomes. Assess food access, home safety concerns related to nutrition, and available support systems that may impact adherence to nutrition recommendations. Document assessments, interventions, care plans, and patient responses accurately and timely within the Electronic Medical Record (EMR) system. Participate in case conferences and interdisciplinary team meetings to support continuity of care. Requirements Bachelor's or Master's degree in Nutrition, Dietetics, or a related field. Current Registered Dietitian (RD/RDN) credential and applicable state licensure preferred. Experience providing nutrition services in home health, community health, hospital, long-term care, or related healthcare settings. Strong knowledge of medical nutrition therapy, chronic disease management, geriatrics, wound care nutrition, and enteral nutrition support. Ability to perform comprehensive nutritional assessments in a home-based setting and develop individualized care plans. Proficiency in EMR documentation and compliance with home health regulatory requirements. Excellent communication, education, and counseling skills with patients, families, and caregivers. Ability to work independently, manage a flexible schedule, and travel to patient homes within the service area. Strong organizational skills and commitment to delivering high-quality, patient-centered care.
