In this vital role, you will develop personalized nutrition care plans, conduct comprehensive patient assessments, and provide expert guidance on dietary management across our rehabilitation facility. Your passion for nutrition and dedication to patient-centered care will help promote optimal health outcomes and empower individuals to achieve their wellness goals. Responsibilities Conduct thorough patient assessments to evaluate nutritional status, medical history, and dietary needs. Develop, implement, and monitor individualized nutrition care plans tailored to each patient's condition and goals. Provide expert guidance on specialized nutrition interventions including tube feeding, weight management, and long-term care strategies. Collaborate with multidisciplinary teams to coordinate care plans, ensuring consistency across medical, nursing, and dietary departments. Educate patients and their families on nutrition principles, lifestyle modifications, and disease management techniques. Utilize electronic medical record (EMR) systems to document assessments, care plans, progress notes, and discharge instructions accurately. Supervise dietetic assistants or interns when applicable, ensuring adherence to best practices and quality standards. Requirements Registered Dietitian (RD) credential from an accredited program with current licensure or certification as required by the state or country. Proven experience in nursing home settings with familiarity in geriatrics and long-term care. Strong knowledge of physiology related to digestion, metabolism, and disease processes affecting nutritional needs. Hands-on experience with dietary management including tube feeding, weight loss programs, and care planning for complex conditions. Proficiency in using EMR systems for documentation and patient tracking. Excellent patient assessment skills with the ability to communicate effectively across diverse populations. Supervising experience or leadership skills preferred for mentoring team members or interns. Knowledge of healthcare regulations related to dietetics practice and safety standards in hospital or clinical settings.
