Job Description

Delaware Oaks Center for Rehabilitation is Hiring!!!
Duties Include:
Integrate the food and nutrition aspects of resident care through communication with the medical and nursing staff. Follows regulations for completing the Minimum Data Set (MDS) as appropriate. Contributes pertinent information to resident care plans and records appropriate assessments, screens and progress notes in the resident's medical record. Provides nutritional counseling and diet education to residents and their families. Implementing of diet changes, modification of menus and food preferences into food service software as needed. Helps the department in reinforcement of policies and procedures and/or training for dietary employees as needed. Attends and participates in resident care plan meetings and develops appropriate integrated care plans. Observes and advises on food preparation, meal rounds as needed. Provides assistance in cost control, food preparation and service, and staffing of the department as needed. Helps the department make routine evaluations to ensure compliance with local, state and federal regulations. Advise physician and nursing supervisor when diet changes or nutritional supplements are necessary. Ensures that physician diet orders are followed for each resident. Communicates additional diet changes to production staff that affects amounts and type of food preparation. Supervises the work of the kitchen and supervisors as needed. Attends administrative staff/department meetings.
Requirements Include:
Bachelor of Science Degree with a major in Clinical Dietetics Preferably Registered Dietitian (RD or RDN) and/or Certified Dietitian of New York (CDN), 1+ year experience (will train the right candidate) A member of the
Academy of Nutrition & Dietetics Preferred Skills & Abilities:
Working knowledge of issues related to food and nutrition Enthusiastic with great interpersonal skills Good time management skills and ability to handle multiple tasks at once Proficiency in email, Microsoft word and electronic medical record entry We offer the
