Job Description

Overview The Clinical Dietitian is responsible for the evaluation, planning and clinical assessment of nutritional status of the residents and participants of Loretto long-term care facilities or sites. Coordinates all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling of residents and participants in a variety of settings. Performs duties in compliance with licensing, accrediting, and regulatory agencies. Contributing member of the interdisciplinary team in establishing and implementing the interdisciplinary care plan. Advise Diet Technician/Home Health Aides regarding food service operations in the Day Center. Attend daily meetings with the team and initial care planning and reassessment meetings. Complete assessment semi-annually and with significant change in status as indicated Active member of tissue trauma team. Adheres to the mission and philosophy of the

Adheres to service excellence by developing and maintaining positive respectful relationships will all customers, internal and external, to include participants, families, team members, all staff throughout the organization and community partners; performs responsibilities according to the highest quality standards. Demonstrates knowledge of
PACE/D&TC
Emergency Preparedness Plan and care verbalize their role during an event. Any other duties required to meet the goals and objectives of the program.

Performs a variety of related duties as assigned. Qualifications Two years of clinical experience in a health care setting desired. Long-term care or outpatient experience preferred. Completion of a dietetic internship. B.A. or B.S. in dietetics from an AND (Academy of Nutrition and Dietetics) accredited academic institution, required. Current registration or registry eligible. Able to effectively assess residents or participants' nutritional needs and establish plans of nutritional care. Ability to work effectively in a collaborative environment. Ability to communicate effectively with participants, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments. Excellent oral and written communications skills required. Proficient in MS Office and able to learn new software rapidly. An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements .
