Job Description

Open Door is currently seeking a Registered Dietitian in our Ossining medical center. This position can be full-time or part-time and range in hours from 22.5 - 37.5 hours per week; schedule is 9AM - 5PM and includes one later shift (11AM-7PM) and one earlier shift (8AM-4PM) per week. The Registered Dietitian participates in a patient-focused, team-based approach to promote health and wellness among patients and staff. As part of the medical care team, the services provided by the Registered Dietitian aim to improve patient health outcomes and knowledge pertaining to nutrition. Primary responsibilities include medical nutrition therapy, nutrition education and nutrition counseling.
DUTIES AND RESPONSIBILITIES
Provide 1:1 and group medical nutrition therapy, nutrition education and nutrition counseling in-person or virtually using voice or video technologies (note: the majority of services are provided in the 1:1 format) Implement the Nutrition Care Process as defined by the Academy of Nutrition and Dietetics' Standards of Care (screening, assessment, diagnosis, intervention, monitoring/evaluation, documentation, outcomes management) Engage patients in the goal setting process and utilize Motivational Interviewing and other techniques to assist in goal achievement; develop nutrition care plans that are individualized as well as culturally and clinically appropriate Collaborate and communicate with other members of the health care team of each patient Provide appropriate evidence/research-based patient education materials when applicable, taking into consideration the patients' learning style and health literacy Make referrals for medical, dental, behavioral health and other relevant services (e.g. care coordination, wellness programs, food access and other community resources) Collaborate with the wellness team on schedule management activities aimed at engaging and retaining patients in care Develop curriculum for, and deliver, medical nutrition therapy group visits, special population nutrition groups, and presentations Perform duties in accordance with established policies and procedures, objectives, quality improvement activities, safety, environmental and infection control standards Maintain electronic medical records in compliance with Open Door's standards Maintain documentation in compliance with The Joint Commission standards and applicable city, state, and federal laws Attends staff and other organizational meetings, as designated, to ascertain Open Door's short and long-range plans, programs, and goals within areas of responsibility Participates in staff development programs, educational activities, seminars, and training opportunities
QUALIFICATIONS EDUCATION
: BS/MS in Nutrition from an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited program required Registered Dietitian (RD)/Registered Dietitian Nutritionist (RDN) required Certified as a Certified Dietitian-Nutritionist (CDN) in New York State required SKILLS Spanish and English proficiency required Excellent written and verbal communication skills including excellent active listening skills Engaging, warm and caring personality Strong customer service, patient-centered orientation Empathetic and driven to assist patients Passionate about providing high quality care to low-income, under-served individuals and those lacking access to health care Able to function effectively as a member of an interdisciplinary team and committed to collaborative team-based care Able to navigate healthcare and community settings as well as patients' natural settings (i.e. place of residence) Culturally competent/sensitive (racial, ethnic, linguistic, LGBTQ+, physical ability, etc.) and committed to inclusive, multi-cultural programming Comfortable providing direct patient care to patients of all ages with chronic medical and behavioral health needs Proficient computer use, including Microsoft 365 applications Able to understand and follow detailed instructions Highly organized and self-directed, able to multi-task Flexible and exhibits the ability to adapt as situations evolve Comfortable working in a fast-paced, high accountability environment Effective in problem solving and exhibits excellent judgment and the ability to prioritize
EXPERIENCE
At least one year experience Experience working in an outpatient community healthcare setting is preferred Experience with electronic medical records is preferred
REQUIREMENTS
Ability to work some evening hours required Must have valid NYS driving license and daily access to a car

Approximately 95% sitting
