Responsibilities:
Provides timely, evidence-based nutritional care and education to patients and families. Participates in the training of medical staff, dietetic interns and peers. Responds to patients' needs and promotes and supports the highest quality nutritional care. Acts as a consultant/resource to all members of the health care team, and serves as liaison/ambassador between patients and foodservice staff.ESSENTIAL FUNCTIONS
Provides ongoing nutritional assessment, monitoring, evaluation, and education to patients and families. Develops and implements individualized nutrition care plans for patients across the lifecycle, from infants to the elderly, as assigned. Integrates evidence-based research in the education, practice and management of patients. Effectively communicates nutrition plans and rationale to team members verbally and in the medical record. Demonstrates the ability to utilize various HIPAA secure patient communication modalities such as Zoom, My Chart, etc. Provides education on medical nutrition therapy and food-drug interactions to patients, their families and significant others. Ensures continuity of care when a patient transitions to an alternate level of care within the facility or at discharge. Develops nutrition education materials. Serves as clinical preceptor to dietetic interns and provides didactic training to nutrition students, peers and other medical professionals. Participates in orienting, training and in-servicing professional and other nutrition staff. Participates in continuing education opportunities. Participates in unit-based, disease-specific or interdisciplinary rounds and acts as a consultant/resource to all members of the health care team. Participates in weekly nutrition support rounds. Serves as liaison/ambassador between patients/units and foodservice staff. Participates in quality assurance activities, including productivity and malnutrition tracking. Serves on committees for department and hospital initiatives. Represents the department at community nutrition functions. Attends regular department staff meetings as scheduled. Other duties as assigned.MINIMUM EDUCATION & EXPERIENCE
Bachelor's degree Accepted if certified by the Commission on Dietetics Registration for degrees received before 1/1/2024 and completion of one-year dietetic internship experience required Master's degree CEND certified for degrees received on/after 1/1/2024 and completion of one-year dietetic internship experience required Or equivalent combination of education and experience Clinical nutrition experience in a healthcare setting preferredKNOWLEDGE, SKILLS AND ABILITIES
Demonstrated strong communication skills required Ability to work independently and effectively in a busy, multi-disciplinary medical practice setting required Ability to prioritize workload and multi-task with accuracy and efficiency requiredLICENSES AND CERTIFICATIONS
