AHC services are not just available to Masons; we are open to the entire community. Per-diem Registered Dietitian position available; $35/hr-$38/hr; $60/visit
JOB SUMMARY
Provides nutritional services to patients and families in the home. The dietician assists the physician and other team members in assessing the nutritional status of the patient. The dietician participates in the development of the plan of care, prepares clinical and progress notes, works with the family, uses appropriate community resources, participates in discharge planning
ESSENTIAL JOB FUNCTIONS
: Completes a comprehensive nutritional assessment to address the patient's needs in The home care setting, implements a process for meeting these designated needs of the patient. Maintains patient and family contact. Advocates for patient family/designated representatives. Utilizes communication skills, both verbal and written, to exercise effective communication within the home care setting. Works with the patient and family to provide nutritional consultation and education. Develops nutritional interventions and participates in developing the plan of care, in collaboration with the patient, family and physician. Maintains and submits records as required, writes progress notes, and documents all interdisciplinary communication. Participates in patient case conferences and Quality Assessment activities. Establishes visit frequencies to meet the patient's needs. Contacts community agencies for services as required. Attends in-service programs as required to maintain registration status and update knowledge. Acts as a consultant to agency staff. Completes initial nutritional assessments, care plans, and appropriate documentation for those patients assigned. Responsible for interdisciplinary communication as needed regarding patient nutritional care. May assist Director of Patient Services and/or Therapy Clinical Manager as requested with policy development, in-service education and quality initiatives. Performs other related duties as assigned.
JOB QUALIFICATIONS
: Bachelor's degree in Nutrition/Dietetics. Past experience in food service. Valid driver's license and/or access to transportation Home Care experience desirable. Desired membership in the American Dietetic Association. The base range for this position is $35-$38 per hour commensurate with work location, experience, skills, certifications, education & prior accomplishments. The position may be eligible for additional compensation not listed here, including but not limited to shift differentials, incentives or bonuses, as applicable.
