Qualifications Dietetics Registered Dietitian Full Job Description Position Summary:
A home health Registered Dietician provides personalized nutritional assessment, counseling, and medical nutrition therapy to the adult and pediatric patients in their homes, aiming to improve health outcomes and manage acute and chronic conditions.
Scheduling:
Monday - Friday, Days 8 am to 430pm
Education:
Bachelor's degree or an equivalent combination of education and experience
Experience:
1-3 years of experience preferred Licensure, Certification, and/or Credential Requirements NYS Registered Dietitian with the Commission on Dietetic Registration Responsibilities Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodation:
Assessment & Care Planning:
Evaluates patient nutritional status, lab values, and weight, creating tailored dietary plans based on diagnoses, food preferences, and cultural backgrounds.
Home Visits and Education:
Conducts in-home counseling for patients and family/caregivers regarding nutrition, food safety, and disease management.
Clinical Interventions:
Addresses malnutrition risk, manages specialized diets (e.g., renal, diabetic, cardiac, wound healing promotion), and modifies diets for issues like swallowing difficulties.
Collaboration and Documentation:
Communicates with providers to adjust plans and documents visits according to agency and regulatory requirements.
Monitoring and Follow-up:
Conducts regular, ongoing assessments to track progress and adjust interventions. Other duties as assigned.
