McGregor PACE is an innovative, community-based Program of All-inclusive Care for the Elderly . Our five senior day centers and clinics are located in the counties of Cuyahoga, Lorain, and Summit. McGregor PACE is expanding! We have opportunities available for an experienced and compassionate Registered Dietitian to work in a community-based setting with the frail and elderly populations. McGregor PACE takes great pride in the provision of comprehensive services for frail older persons. The Dietitian provides direct participant care to program enrollees; develops and implements nutritional care plans, communicates plans, provides education to participants, family, and staff as needed, establishes a follow-up system and maintains documentation. The Dietitian works with an Interdisciplinary Team to develop plans of care that meet the participants' needs. The workplace setting includes our PACE senior day centers and clinics, and home visits which may include visiting assisted living facilities, long-term care facilities, group homes, and community homes. There is also flexibility for remote work!
Qualifications include:
Registered Dietitian; licensed as a clinical dietitian in the state of Ohio Bachelor's degree in nutrition and dietetics, required; master's degree, preferred Minimum of one year's experience working with a frail or elderly population, required Experience in community-based programs and interdisciplinary care settings, desirable Serv Safe Certification desired Current Ohio driver's license and reliable transportation, required We offer an excellent benefits package that includes: Paid Time Off Sick Time 6 Paid Holidays Health, Dental Vision Basic Life Voluntary Life STD LTD 403b Retirement Plan Health Savings Account.
