Sweetwater Care is seeking a Registered Dietitian to join our team in a skilled nursing setting. Core Responsibilities:
Complete timely, accurate nutrition assessments, follow-ups, and documentation in accordance with CMS regulations and facility policies Develop and implement individualized nutrition care plans that align with physician orders and resident goals Monitor residents at nutritional risk, including weight changes, wounds, tube feeding, and altered intake Collaborate with nursing, therapy, food service team, and medical providers to ensure coordinated care Ensure diet orders are accurate, implemented correctly, and communicated effectively to the food service team Support survey readiness through compliant documentation and participation in audits as needed Provide education to residents, families, and staff as appropriate Participate in Quality Assurance and Performance Improvement (QAPI) activities
Qualifications:
Must be a Registered Dietitian with CDR (Commission on Dietetic Registration) Must hold a state license in the state of which you are applying if indicated 1 Year experience preferred
