Full Time (30 Hours or More) Job Requirements and Properties Work Onsite Full Time Schedule Full Time Job Description . Overview This position is eligible for a $7500 Sign on/Retention Bonus. Evaluates, assesses, and plans nutrition care to meet patient nutritional needs. Responsibilities- Assesses patient nutritional status using documented medical information and established nutritional standards.
- Counsels patients and/or families by identifying nutrition-related problems. Develops plans of instruction and nutritional care, and monitoring results.
- Identifies inpatients at nutritional risk and documents recommendations.
- Diagnose and develop intervention for patients with malnutrition
- Participates in physicians' rounds, discharge planning rounds, and conferences with nutritional implications.
- Conducts education programs for patients, community groups, and staff.
- Precept and mentor new staff or existing staff in clinical specialty area.
- Develop therapeutic relationships with multiple subspecialty care team members and providers that enables planning and participating across a broader spectrum patient care.
- Cross-covers a broader spectrum of patient care areas and subspecialties as needed.
- Performs other duties as required or assigned. Qualifications
- Bachelor's Degree from an accredited program in nutritional sciences (prior to 2024).
- Master's Degree from an accredited program in nutritional sciences (2024 and after).
- Minimum 2 years' related experience preferred. Required Licensure/Certifications
- Current NH Dietitian license required. May require additonal license if providing Telehealth services.
- Current Registration by the Commission on Dietetic Registration.
