Patriot Dialysis LLC Overview Join our dynamic healthcare team as a Registered Dietitian, where your expertise will empower patients to achieve optimal health through personalized nutrition strategies. In this vital role, you will assess individual dietary needs, develop comprehensive care plans, and provide expert guidance to promote wellness across diverse populations. Duties Conduct thorough patient assessments to evaluate nutritional status and identify specific dietary needs. Develop and implement tailored nutrition care plans that align with medical diagnoses, treatment goals, and individual preferences. Provide expert counseling on weight management, disease-specific nutrition (such as ESRD and diabetics), and lifestyle modifications through health coaching. Manage specialized nutritional interventions and diagnostic evaluation of nutritional deficiencies. Utilize Electronic Medical Record (EMR) systems to document assessments, care plans, progress notes, and discharge summaries accurately. Collaborate with multidisciplinary teams including physicians, nurses, and therapists to optimize patient outcomes across hospital and long-term care environments. Skills Strong knowledge of physiology relevant to nutrition, metabolism, and disease processes. Expertise in developing care plans for diverse populations including dialysis patients requiring specialized diets. Skilled in conducting patient assessments, diagnostic evaluations, and creating individualized nutrition strategies. Experience with EMR systems used within hospital or healthcare settings for documentation and communication. Deep understanding of acute care environments, long-term care facilities, and dietary department operations. Ability to deliver engaging health coaching focused on weight loss, lifestyle changes, and overall wellness initiatives.
