  • Contract Header Overview Join our dynamic team as a Registered Dietitian, where you will play a vital role in promoting optimal nutrition and health outcomes for diverse participant populations.
This position offers an exciting opportunity to apply your expertise in dietetics and care planning to improve quality of life across various settings. As a key member of our multidisciplinary team, you will deliver personalized nutrition assessments, develop tailored care plans, and provide expert guidance on dietary management. Your passion for participant-centered care and your knowledge of long-term care will make a meaningful difference every day. A. Qualifications 1. The dietitian is registered by the Commission on Dietetic Registration and has provided the center with a copy of his/her registration. 2. The dietitian will provide the center with a health examination, signed by a physician, and a TB clearance, which is not more than 12 months prior to employment or within 7 days of employment and shall provide the center a current certificate of CPR and First Aid training. B. Duties The dietitian is responsible for, and supervises the provision of all nutrition services, as specified in Title 22, California Code of Regulations, and Sections 78319 through 78333 and 54331. The duties of the dietitian include: 1. Designing the nutrition services program for the center and defining the responsibilities of the dietitian and any staff providing nutrition services. 2. Providing consultant services on the center premises on a regularly scheduled basis and maintaining a written record of the frequency, nature and duration of the visits/services. 3. When at center, supervising the center staff during meal preparation to ensure appropriate measuring utensils are used, to ensure staff are using hairnets and gloves, and to ensure proper procedures are followed in the kitchen area. 4. Attending and participating in all the multidisciplinary team (MDT) meetings and subsequent MDT meetings if dietitian services are provided by the dietitian or recommended by the RN or physician. 5. Conducting an initial assessment of any participant referred for evaluation, and subsequent reassessments at least every 90 days if nutrition services are being provided, including participants with therapeutic diet orders. 6. Developing the plan of care in collaboration with the participant's identified goals and/or concerns. 7. Completing, for each participant, at minimum, an initial assessment, quarterly and six-month reassessments which includes evaluating and reviewing the progress made towards the participant's identified goal(s) collaboratively with participant and updating the plan of care as needed or desired by the participant and the collaborative process. a. Additional reassessments/updating of the IPC may also be completed in addition to the quarterly and six-month reassessments for the following reasons: when participant or his or her representative request, following inpatient hospital stay, after being away from the center following 30 days absence or a significant change in the participant's status, such as new diagnosis, new home environment, caregiving support, increased impairment/concerns in participant's mental/physical/emotional symptomology/status. 8. Providing all nutrition services specified as being the dietitian's responsibility in the plans of care, including dietary counseling and education. 9. Reviewing all therapeutic diet orders every 90 days. 10. Approving menus for all meals, between meal nourishments, and therapeutic diets. 11. Coordinating with the meal provider to develop the menus and approve substitutions for all meals and snacks. 12. Maintaining the nutrition services component of the participant health records, if nutrition services are being or have been provided.
This includes:
a. Documentation of all nutrition services provided, including the participant's reaction to treatment, on the day the service(s) is given. b. Documentation of all initial assessments and, at least every six-months, reassessments completed for nutrition services. c. Documentation of all individual plans of care developed for nutrition services. d. Documentation of any marked changes in a participant's condition or unusual incidents reported to or observed by the dietitian. e. If nutrition services are being provided and are subsequently discontinued, documentation of the grounds for taking this action. 13. The dietitian works the hours necessary to meet the needs of the participants based upon their plans of care, to meet the requirements specified in Title 22, California Code of Regulations. 14. The dietitian reports to the Program Director. 15. Will be knowledgeable of
ADHC/CBAS
laws, regulations, CBAS certification standards, standards of participation and policies and procedures. This will be ensured through ongoing staff training and education.
