Job Description

Join a Different Kind of Healthcare Team Creekside Private Care is a boutique concierge nursing and care coordination practice serving a limited number of affluent families throughout Scottsdale, Paradise Valley, Phoenix, and surrounding communities. Unlike traditional home care agencies, we focus on highly personalized, relationship-based support for individuals living with chronic illness, dementia, Alzheimer's disease, complex medical conditions, and those requiring end-of-life support. Our interdisciplinary team includes nurses, social workers, chaplains, physician oversight, and care coordination professionals who work together to deliver an exceptional level of service. We are seeking a compassionate and experienced Registered Dietitian/Nutritionist to join our team on a PRN basis as we continue to grow. Position Summary The Dietitian/Nutritionist will provide individualized nutritional assessments, meal planning recommendations, caregiver education, and ongoing nutritional support for clients with complex health needs. This role will work closely with our nursing team, physician, social workers, and family members to promote comfort, wellness, symptom management, and quality of life. This position is ideal for a clinician who values meaningful patient relationships, autonomy, flexibility, and the opportunity to practice without productivity quotas or high-volume caseloads. Responsibilities Conduct comprehensive nutritional assessments in clients' homes Develop individualized nutrition and hydration plans Provide recommendations for chronic disease management through nutrition Support clients with: Dementia and Alzheimer's disease Cancer diagnoses Heart failure and cardiac conditions Diabetes management Kidney disease Weight loss and malnutrition Dysphagia and altered diets End-of-life comfort care Collaborate with nursing staff and physician oversight regarding nutritional concerns Educate family members and caregivers on dietary needs and meal planning Assist with nutritional interventions following hospitalization Participate in interdisciplinary care planning when appropriate Document assessments and recommendations in accordance with agency standards What Makes This Role Different Boutique, low-census model Private-pay clientele No productivity quotas Flexible scheduling Collaborative interdisciplinary team Opportunity to build meaningful relationships with patients and families Focus on quality of care rather than volume Supportive startup environment with significant growth potential Qualifications Registered Dietitian (RD/RDN) credential required Current Arizona licensure (or eligibility) Minimum of 2 years of clinical experience preferred Experience with geriatrics, dementia care, hospice, palliative care, home health, or chronic disease management strongly preferred Excellent communication and family education skills Ability to work independently in home settings Reliable transportation and valid driver's license Compensation Competitive per-visit compensation Mileage reimbursement Flexible PRN scheduling Opportunity for expanded hours as census grows Ideal Candidate We are looking for someone who believes healthcare should be personal, relationship-driven, and centered around dignity.
