In this vital role, you will provide expert nutritional guidance and personalized care plans to diverse patient populations. This position offers an exciting opportunity to make a meaningful difference in patients' lives by promoting healthy eating habits, managing complex medical conditions, and guiding recovery processes. The hiree will create diet plans and will follow up on patients monthly Responsibilities Conduct thorough patient assessments to evaluate nutritional needs, medical history, and physiological status. Develop and implement individualized nutrition care plans aligned with diagnostic evaluations and treatment goals. Provide specialized counseling on weight loss, disease management, tube feeding, and dietary modifications for various conditions. Collaborate with multidisciplinary teams to coordinate care strategies within hospital or long-term care settings. Supervise and mentor dietetic interns or junior staff members to ensure quality standards are maintained. Utilize electronic medical record (EMR) systems to document assessments, interventions, progress notes, and discharge planning. Educate patients and their families on nutrition principles, lifestyle changes, and ongoing health maintenance strategies. Requirements Valid registration as a Registered Dietitian Nutritionist (RDN) with current licensure or certification. Proven experience in hospital settings or long-term care facilities with a focus on geriatrics and acute care. Strong knowledge of physiology related to digestion, metabolism, and nutritional requirements across age groups. Hands-on experience with dietary management techniques such as tube feeding and specialized diet plans. Demonstrated skills in health coaching, patient assessment, and developing effective care plans. Supervising experience is preferred, along with the ability to lead a team within a fast-paced healthcare environment. Familiarity with EMR systems for accurate documentation and communication of patient information. Excellent communication skills to effectively educate patients about nutrition strategies for weight loss, chronic disease management, or recovery phases.
