Job Description

The primary purpose of the Clinical Dietitian is to provide nutrition care for all residents in all assigned areas according to established practice guidelines and Nutrition Services policies along with current federal, state, and local standards. This position collaborates and coordinates resident care with members of the interdisciplinary health care team. The Clinical Dietitian reports directly to the Chief Clinical Dietitian.
Education Minimum Education:
Bachelor's Degree and/or Master's Degree Minimum years of
Experience:
Prior knowledge of nutrition assessment and documentation in a long term care setting desired.
Required Certifications/Licensure:
Registered (through examination administered) by the Commission of Dietetic Registration of the Academy of Nutrition and Dietetics; Current license to practice dietetics from the state of South Carolina. (Please note: Registry eligible candidates may be considered with the following provisions: Must provide documentation from Commission of Dietetic Registration of eligibility status, and must complete registration exam and obtain a license to practice dietetics in South Carolina within 6 months of start date as a condition of continued employment). Major Duties & Responsibilities 1. Demonstrates competence to provide developmentally appropriate care/treatment for the following resident population: Adult (18 years to 65 years) Geriatric (over 65 years) 2. Provides input and is active in the development of nutrition screening/assessment parameters. 3. Provides input and is active in the development of nutrition protocols and policies. 4. Collaborates coordinates with the resident, family member, and/or caregiver to assess needs, set priorities, establish goals/outcomes and develop individualized care plans. Continues to monitor progress toward achieving goals/outcomes. 5. Interviews residents/family members/care givers to obtain diet history and food preferences. Routinely updates resident's food preferences in menu system. 6. Participates in department and hospital-wide performance improvement data collection. Provides input into department's performance improvement plan of action for improvements. 7. Practice reflects best evidence, applicable laws, and other regulatory standards in the provision of nutrition services. 8. Performs the key functions of the nutrition care process according to policy. 9. Uses accurate and relevant data and information to identify nutrition-related problems. 10. Identifies and labels specific nutrition problem(s) that will be addressed in the plan of care. 11. Identifies and implements planned interventions with the intent of changing or improving a nutrition related behavior, risk factor, or condition. Interventions may include resident/family education. 12. Monitors and evaluates data directly related to the nutrition diagnosis, goals, and interventions to determine progress made in achieving desired outcomes of nutrition care. 13. Participates in resident's discharge planning as needed. 14. Manages weight program in assigned areas. 15. Completes assigned monthly, quarterly and annual nutrition documentation in a timely manner. Monthly charting includes nutrition wound management, tube feeding, dialysis reviews and significant / non-significant weight loss evaluations.
