Job Description
POSITION SUMMARY
The Registered Dietitian/Nutritionist (RD/N) supports the medical care team by providing comprehensive nutritional assessment, education, and medical nutrition therapy to medically underserved patients with chronic conditions, with a focus on individuals who are overweight or obese and at risk for disease progression, complications, and cognitive and functional decline. In collaboration with primary care providers and the interdisciplinary care team, the RD/N develops and implements individualized, culturally responsive, patient-centered care plans that promote disease prevention and self-management, improve access to nutrition care, enhance quality of life, and support improved clinical outcomes.ESSENTIAL DUTIES/RESPONSIBILITIES
Conduct comprehensive nutritional assessments, nutrition education, counseling, and medical nutrition management for patients referred by providers and/or interdisciplinary care team members. Provide individualized instruction to patients regarding modified diets and therapeutic nutrition interventions related to health promotion, obesity prevention, and chronic disease management. Coordinate, implement, and evaluate nutrition education programs in accordance with current evidence-based practice guidelines, national standards, and regulatory recommendations. Evaluate patients for needed preventive and condition-specific services, including support of Uniform Data System (UDS) and Healthcare Effectiveness Data and Information Set (HEDIS) quality measures, and assist in closing identified gaps in care. Deliver structured nutrition counseling and lifestyle intervention services for patients with diabetes, obesity, hypertension, cardiovascular disease, and other chronic health conditions. Monitor, track, and document Body Mass Index (BMI), weight trends, nutritional status, patient progress, and lifestyle modification interventions within the electronic health record (EHR). Develop culturally and linguistically appropriate educational materials, programs, and teaching tools to support patient understanding of nutrition and healthy lifestyle practices among diverse populations. Provide outpatient and community-based nutrition education and counseling services to patients, families, and groups in accordance with individualized care plans and organizational goals. Prepare and deliver nutrition-related presentations, workshops, and wellness education sessions to patients, staff, and community stakeholders. Maintain timely, accurate, and complete clinical documentation, including assessments, care plans, interventions, follow-up activities, and billing documentation in accordance with organizational policies and payer requirements. Collaborate with providers and interdisciplinary care team members to update and revise nutrition care plans based on patient progress, identified risks, and clinical outcomes. Actively participate in medical staff meetings, interdisciplinary care team conferences, daily huddles, quality improvement initiatives, and department-specific meetings. Maintain current Basic Life Support (BLS) certification, recognizing the role functions within a direct patient care environment. Perform additional duties, special projects, and responsibilities as assigned to support organizational operations and patient care initiatives.EDUCATION/EXPERIENCE
Bachelor's degree in Dietetics, Nutrition, or a related field from a program accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) or other accrediting body recognized by the Academy of Nutrition and Dietetics. Current registration as a Registered Dietitian/Registered Dietitian Nutritionist (RD/RDN) through the Commission on Dietetic Registration (CDR), including successful completion of the national registration examination. Current licensure or eligibility for licensure as required by the State of Tennessee. Demonstrated experience working with both adult and pediatric patient populations. Knowledge of evidence-based nutrition guidelines and medical nutrition therapy related to chronic disease prevention and management, including obesity, diabetes, hypertension, and cardiovascular disease. Working knowledge of medical terminology, clinical documentation, and healthcare practices. Strong interpersonal, organizational, communication, and patient education skills. Ability to work collaboratively within an interdisciplinary healthcare team in a Federally Qualified Health Center (FQHC) or community health setting preferred.Performance Skills:
Demonstrated ability to work independently while developing and maintaining productive, collaborative relationships with providers, interdisciplinary care team members, and community partners. Ability to recognize and address the cultural, linguistic, health literacy, developmental, and age-specific needs of diverse patients, families, and caregivers. Advanced patient interviewing, assessment, motivational interviewing, and counseling skills. Strong interpersonal, verbal, and written communication skills with the ability to effectively educate and engage patients and multidisciplinary teams. Excellent critical thinking, problem-solving, analytical, organizational, and decision-making abilities. Current knowledge of evidence-based standards of care, clinical practice guidelines, and medical nutrition therapy for chronic disease prevention and management. Ability to work autonomously, manage schedules across multiple clinic sites, prioritize responsibilities, and maintain productivity through proactive patient outreach, referrals, and case identification. Proficiency in computer applications and healthcare technology, including Microsoft Office Suite (Word, Excel, Outlook), electronic health records (EHR), patient registries, telehealth platforms, and population health management tools. Ability to maintain accurate clinical documentation and comply with organizational, regulatory, and billing requirements.Pay:
$60,000.00 - $72,000.00 per yearBenefits:
