Full Job Description Overview:
In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for planning, coordinating and evaluating nutrition and dietary services and programs.Responsibilities:
Essential Duties:
Evaluate the dietary needs of patients, develop and implement a dietary plan, and document nutritional information in patient medical charts. Educate patients on food selection to maintain and promote health as well as to prevent and treat illness and disease. Develop and conduct specialized patient nutrition classes and individual patient instruction. Provide information and education to staff members using nutrition education materials or in-service/meetings per request. Implement and monitor quality control programs. Evaluate programs for effectiveness and accomplishment of goals. May be responsible for educational instruction or instructional team participation for the DMST program. May participate in or manage special projects/programs as needed. Perform other duties as assigned.Primary Accountabilities:
Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures. Develop and maintain favorable internal relationships, partnerships with co-workers, including clinical managers, clinical support staff, providers and business office staff. Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families. Collaborate with all members of the care team in providing patient-centered care. Ensure all actions, job performance, personal conduct and communications represent CommUnityCare in a highly professional manner at all times. Uphold and ensure compliance and attention to all company policies and procedures as well as the overall mission and values of the organization. Complete all mandatory training and maintain appropriate credentials/licensure.Knowledge/Skills/Abilities:
High level of skill at building relationships and providing excellent customer service. Basic computer systems knowledge, including but not limited to Microsoft Office products. Strong attention to detail and accuracy. Excellent verbal and written communication skills. Ability to apply interpretations of federal, state and local regulations. Ability to examine and evaluate the need for nutrition and dietary services and programs. Bilingual (English/Spanish) desirable.Qualifications:
MINIMUM EDUCATION
Bachelor's degree in Dietetics.MINIMUM EXPERIENCE
Experience providing nutritional services in an inpatient/outpatient, one-on-one or group education setting. Demonstrated knowledge of foods, nutrition and food systems management. Extensive knowledge of available resources in the field of nutrition education and dietary services. Experience developing and conducting on-going in-service training programs. Experience developing and conducting specialized nutrition classes/individual patient instructions. Experience in program/project management desirable.PREFERRED EXPERIENCE
REQUIREDCERTIFICATIONS/LICENSURE
Holds and maintains these certifications as a professional. Lapsing/expiration of these certifications/licensure will result in suspension of work: 1. Current license in the state of Texas as a Registered Dietitian (RD).REQUIRED CERTIFICATIONS/LICENSURE
