Job Overview We are seeking a passionate and dedicated Clinical Nutritionist to join our dynamic healthcare team. In this vital role, you will provide expert nutritional guidance and personalized care plans to diverse patient populations in functional medicine health plans . Your expertise will help optimize patient health outcomes through comprehensive assessments, tailored nutrition strategies, and ongoing support. This position offers an exciting opportunity to make a meaningful difference in patients' lives by promoting wellness, recovery, and long-term health maintenance. Duties Conduct thorough patient assessments to evaluate nutritional status, medical history, and dietary needs using diagnostic evaluation techniques. Develop individualized care plans that incorporate evidence-based nutrition interventions aligned with patient goals and medical conditions. Provide expert guidance on specialized nutrition therapies such as weight loss programs, and dietary modifications for chronic illnesses. Collaborate with multidisciplinary teams including physicians, nurses, functional medicine providers, and health coaches to ensure cohesive patient care. Monitor patient progress regularly through follow-up consultations and adjust nutrition plans accordingly to maximize health benefits. Educate patients on proper nutrition practices, lifestyle changes, and the importance of sustained healthy habits. Utilize Electronic Medical Record (EMR) systems efficiently to document assessments, care plans, and progress notes accurately. Experience Proven experience in clinical nutrition or dietetics within healthcare settings. Strong knowledge of physiology related to digestion, metabolism, and disease processes affecting nutritional needs. Hands-on experience with dietary department operations and managing nutrition services in long-term care or acute care environments. Demonstrated proficiency in patient assessment techniques and developing effective care plans for diverse populations. Familiarity with EMR systems for documentation and communication within healthcare teams. Prior experience in weight loss programs or dietary interventions aimed at chronic disease management is highly desirable. Pay:
$23.00 - $28.00 per hour
