Responsibilities Registered Dietitian (RD / CDN): Clinical Nutrition & Resident Care Complete nutrition assessments and reassessments for residents Develop, implement, and monitor individualized nutrition care plans Provide nutrition education and counseling to residents and families Monitor residents' nutritional status, weight trends, intake, and overall wellness Maintain accurate nutrition documentation within the electronic medical record system Collaboration & Team Partnership Participate in interdisciplinary care plan meetings Partner with Dining Services to support appropriate meal planning and service Serve as a nutrition resource for facility leadership and clinical teams Support and mentor Diet Technicians and dietetic interns, as applicable Quality, Compliance & Education Conduct nutrition-related audits and performance improvement initiatives Maintain compliance with CMS, DOH, and nutritional service regulations Assist with staff education and in-service training programs Support survey readiness and best practice initiatives From Up to Qualifications Registered Dietitian (RD / CDN): Bachelor's degree in Nutrition, Dietetics, or related field Registered Dietitian credential required Active registration with the Commission on Dietetic Registration (CDR) Certified Dietitian-Nutritionist (CDN) or state licensure, as applicable Long-term care, rehabilitation, or geriatric experience preferred Strong communication and interdisciplinary collaboration skills Experience with nutritional software and electronic medical records preferred The Right Candidate Will Bring Compassionate, resident-centered approach Strong clinical assessment and critical thinking skills Excellent communication and counseling abilities Team-focused and collaborative mindset Strong organizational and documentation skills Passion for improving resident health and quality of life This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
