reviews $40.80 - $61.83 an hour - Full-time Job Summary Coordinates all phases of nutritional care including screening, assessing, monitoring, educating and documenting. Responsible for practicing and ensuring adherence to SJH Corporate Compliance Program. Provides age and developmentally appropriate clinical care and optimal nutrition information to all populations. Respond to department requests from other areas and ensure appropriate staff manage those requests. Attend meetings as needed as well as maintain policies and guidelines. Responsible for assuring departmental readiness for all State Surveys, TJC, and other licensing, accrediting, and regulatory agencies. Contributes to department budget creation. Adheres to and practices the SJH Corporate Compliance Program and participates in Performance Improvement activities. Essential Functions Clinical Care Uses the Nutrition Care Process to accurately and consistently assess patients and formulate a plan of care. Functions as a member of the clinical interdisciplinary team. Communicates any recommendations or issues with clinical staff and documents all interventions in the medical record. Communicates diet restrictions/special diets to patients and room service staff. Responsible for sharing weekend call duty with other dietitian(s) for all patient care areas, including assessment of all nursing screens and completion of all weekend consults. Screens status of all newly admitted patients according to Nutrition Services Department policies. Reviews patient clinical status in EHR, screens those at nutritional risk and documents all nutritional screens in the patients' medical record. Completes timely assessments of all patients identified to be at high nutritional risk and documents in HER. Critical Care and Specialty Clinic Support Formulates specialized diets including tube feeding (TF) and total parenteral nutrition (TPN). Works with physicians and pharmacists to assure TF and TPN provide optimal nutrition support. Completes high risk assessments in a timely manner. Provides consults to the Oncology department and keeps oncology education information updated. LTC Competence Be able to screen, monitor, assess and care plan for LTC residents, and skilled/IRF patients according to standards of care and regulatory agencies. Participate in care conferences, monthly chart checks and weekly reviews of nutritionally at-risk residents/patients. Updates diet sheets and assures that resident information and menus are accurate and appropriate. Provide diet consultations to skilled and LTC population. Other Duties Lead Food Services education and create staff meeting agendas. Monitor and maintain supplements and ordering as well as menu carb counts and diet manuals. Participates on committees and attends meetings as requested. Works on Quality and PI projects, assuring that the Nutrition Services department meets all state and DNV survey standards. Updates policies. Function as the liaison to the dietary staff to answer questions, provide education around special diets or participate in training as requested. Responsible for the scheduling and coordination of clinical dietitians to assure adequate coverage of all patient care areas including weekend call for Clinical Nutrition Services for all patient care areas. Participation on committees, attending meetings as requested, PI projects, assuring that Nutrition Services meet all State and DNV survey standards. Collaborate on projects that may involve numerous departments or community leaders. Able to cross cover and back fill the outpatient clinical dietician if needed. Skill Development Maintains, develops and updates skills to implement ideal care and service. Keeps licenses and certifications current. Helps identify relevant training for clinical staff. Minimum three years clinical experience. Basic computer skills. Good communication, interpersonal, phone and customer service skills. Ability to adjust to fluctuating job demands, meet assigned deadlines and work independently.Required Licenses:
Registered Dietitian with the Academy of Nutrition and Dietetics. Licensed Dietitian with the State of Wyoming. Functional Demands W orking Conditions Walking, standing and sitting at a computer.
