Graduate of an accredited school of Nursing 2. Possesses a valid and current RN/LVN license 3. 1-2 years experience in long term care preferred 4. Possesses effective written and verbal communication skills 5. Good physical and mental health 6. Must be able to perform any physical requirement of the position which includes lifting dead weight or unassisted weight of 40 pounds. 7. Is capable of following oral and written instructions 8. Has training, knowledge and experience which allows for general supervision of responsibilities 9. Is capable of working independently 10. Has excellent time management abilities 1. Completes MDS Assessments for HMO, Medi-Cal, and private pay residents according to OBRA Regulations. 2. Identifies the triggers and completes the R.A.P. summary. 3. Initiates the residents interdisciplinary care plan according to the R.A.P. summary, current diagnosis and physician orders. 4. Completes quarterly assessments. 5. Assists in the coordination of plan of care by following up on needs identified related to the assessment. 6. Reviews and updates care plans quarterly. 7. Completes Discharge Tracking for HMO, Medi-Cal and private pay residents according to OBRA Regulations. 8. Coordinates MDS Assessment Reference Dates with IDT Members and maintains log as directed by Director of Nursing Services or Administrator. 9. Ensures appropriate scheduling of Care Conferences and communicates the schedule to IDT Members at least 2 weeks prior to meeting. 10. Leads the Care Conference meeting and ensures Residents are fully discussed as an Interdisciplinary Team. 11. Participates as an Interdisciplinary Team member as directed by the Director of Nursing Services. 12. Complies with Facility Polices and Procedures. 13. Attends in-service meeting as directed. 14. Serves as a role model for nursing staff. 15. Promotes a positive, calm and effective work environment. 16. Will provide direct patient care as needed and as directed by the Director of Nursing Services 17. Serves on committees as assigned by the Director of Nursing Services. 18. Works hours approved by the Director of Nursing Services and/or designee. 19. Other tasks as assigned by the Director of Nursing Services or Administrator.
