Clinical Resource

Position Overview:

This position is responsible for providing clinical resources, mentoring, training and education, and clinical practice guideline implementation support to facility leadership, interdisciplinary team (IDT) members, and facility staff to support overall clinical outcomes for facilities and the Market.

Essential Duties and Responsibilities:

Provides relevant and accurate information so that the facility can make informed decisions from a variety of alternatives. Assists facility leadership in achieving and maintaining clinical standards and regulatory compliance (e.g., Clinical Systems Reviews, mock surveys). Assists in developing and conducting required in-service education programs. Assists with quality improvement and QAPI processes. Evaluates the effectiveness of training, education, and the facility's QAPI processes and outcomes. Serves as an Educator for nursing and ancillary staff. Analyzes facility quality outcome reports (QM, eMPR data, Five-Star Ratings, survey results, and other applicable clinical metrics in Power BI) on a monthly and quarterly basis. Provides summary reports of services to the Director of Nursing and Administrator. Assists facility leadership in coordinating improvement initiatives and projects with the Quality Assurance and Performance Improvement (QAPI) Committee. Participates in the implementation of the 100-Day Clinical Plan for newly acquired facilities and facilities with newly transitioning Directors of Nursing. Submits formal or informal monthly progress reports to the Clinical Market Leader. Collaborates with Market IDT resources at least monthly. Develops collaborative partnerships with other Clinical Nurse Resources and IDT resources. Performs other duties as assigned by the Clinical Market Leader.

Qualifications:

Registered Nurse with a current and active California license. Baccalaureate degree in Nursing preferred. At least five (5) years of experience in long-term care or related fields. Previous experience as a DNS in skilled nursing facility. Proficiency in PCC. Experience in facility operations. Demonstrates effective collaboration with others and the ability to assume a leadership role. Demonstrates proficiency in operating computer systems and navigating the Electronic Health Record (EHR). Demonstrates the ability to adapt to changing technologies used for patient care, documentation, and clinical performance outcomes. Experience in quality improvement. Extensive knowledge of current federal and state nursing home regulations and legislation. Knowledge of Infection Prevention and Control (IPC) programs in nursing homes. Successful completion of the Nursing Home Infection Preventionist Training provided by the CDC (effective June 1, 2020) or the

CDPH HAI

Program Infection Preventionist Training for Skilled Nursing Facilities. Experience in multi-facility oversight role preferred. Ability to travel up to 80% of the time. Flexible with work hours to meet the market clinical needs. Able to prioritize and organize tasks at hand to meet specific deadlines. Proficient in Microsoft Word, Outlook, and Excel. Knowledge of state and federal regulations.

Pay:
