We work closely with primary care physicians, specialists, therapy teams, and facility staff to manage transitional care and rehabilitation with a strong focus on continuity, quality, and outcomes. We partner with facilities throughout Colorado, including long-term care centers, skilled nursing facilities, and rehabilitation centers. Our patient-focused team is committed to providing personalized, compassionate care to individuals recovering from serious illness, injury, or surgery. Our providers play an important role in simplifying what can often be a complex transition—from hospital discharge to rehabilitation and, ultimately, back home. Position Summary We are seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing team. The ideal candidate will have experience in hospital medicine, skilled nursing, long-term care, rehabilitation, or a related setting. This role requires strong clinical judgment, excellent communication skills, and the ability to work collaboratively with patients, families, physicians, and facility staff.
