Job Description
Job Overview We are seeking a dedicated and experienced Director of Nursing/Clinical Manager lead our care team in providing exceptional care. The ideal candidate will possess a strong background in nursing administration and acute care, with a passion for improving patient outcomes and ensuring patient rights are upheld. This leadership role requires a commitment to fostering a supportive environment for both staff and home bound patients while overseeing the development and implementation of care plans. Duties Oversee the care team, ensuring compliance with all regulatory standards and best practices. Supervise care staff, providing guidance, training, and performance evaluations. Develop, implement, and evaluate individualized care plans for patients. Ensure that patient rights are respected and upheld at all times. Collaborate with interdisciplinary teams to promote optimal patient care and outcomes. Maintain up-to-date knowledge of nursing practices, physiology, hospice care, and home care services. Lead initiatives aimed at improving quality of care within the facility. Recruit for care team to ensure that business has enough care staff to be able to ensure adequate coverage and support for our patients. Experience Proven experience in nursing leadership or administration within a Home Health Agency Experience in a Medicare, Medicaid and Insurance setting. Strong knowledge of patient's rights, nursing regulations, and best practices in patient care. Experience in supervising care staff and managing teams effectively. Familiarity with developing care plans tailored to individual patient needs. Background in hospice care or home care services is a must. Excellent communication skills with the ability to foster relationships with staff, patients, and families.
