Job Description

1. Responsible for knowing and enforcing nursing policy and procedures of Prairie Living at Chautauqua and all applicable state regulations. 2. Responsible for the assessing and monitoring of health and social needs of residents to support individualized quality of life. 3. Coordinates and assists with the development of Individual Service Plans (ISP) as defined by regulations. 4. Provides support and over-sight to ensure the care teams are following the ISP as developed. 5. Maintains resident health status records, with the Executive Director, in compliance with internal policies of Prairie Living at Chautauqua and all applicable state regulations. 6. Reviews "hot rack" charting daily and coordinates with floor nurse to ensure appropriate coordination and implementation of changes with resident's personal physician and
HCPOA. 7.
Monitors and recognizes resident condition changes through daily rounds and ensures appropriate actions are taken to coordinate care to meet the changing needs, whether acute or permanent. 8. Responsible for all aspects of medication administration and the appropriate use of psychotropic medications and narcotics, ensuring all policies and procedures are strictly adhered to by all nursing staff. 9. Monitors resident's weights and works with dietary for resident's specific needs. 10. Monitor infection control procedures to ensure compliance with all staff. 11. Maintains on-going communication with the Executive Director regarding the well-being of residents. 12. Monitors and orders nursing supplies and personal care items. 13. Identifies and coordinates both internal and external health care services for each resident as need arises. 14. Develops and maintains excellent family relationships. 15. Works closely with other administrative team members to ensure quality of care and performs other duties as needed and requested.
