Key Responsibilities Clinical Leadership & Oversight Partner with the Director of Nursing to oversee clinical operations and ensure compliance with all state, federal, and accreditation standards Lead, mentor, and evaluate licensed and unlicensed professionals to ensure safe, high-quality, and patient-centered care Conduct competency evaluations and ensure appropriate staff assignments for optimal outcomes Perform supervisory visits, reassessments, and clinical oversight in home settings Patient Care & Case Management Manage a designated caseload across service territories while maintaining strong clinical presence Perform comprehensive assessments and develop individualized, goal-oriented care plans Deliver skilled nursing care, including wound care, IV therapy, infusions, injections, and medication management Educate and support clients and families to promote independence and improved health outcomes Serve as a trusted clinical resource and primary point of contact for clinicians, clients, and families Operations, Recruitment & Quality Improvement Ensure timely, accurate, and compliant documentation within EMR systems Maintain strict adherence to HIPAA and confidentiality standards Drive quality assurance and performance improvement initiatives Support implementation of innovative clinical programs and best practices Collaborate cross-functionally with leadership, HR, and operations to enhance service delivery Interview and recruit licensed professionals to meet the growing needs of the business and ensure appropriate staffing levels Community Engagement & Growth Represent BrightStar Care within the healthcare community to build strong referral relationships Support the growth of home care and healthcare staffing services Qualifications Active, unrestricted Registered Nurse (RN) license in Illinois BSN required Minimum 3+ years of experience in home health, case management, or related field Previous leadership or supervisory experience preferred Home infusion experience strongly desired Understanding of IDPH, Joint Commission, and regulatory compliance standards Demonstrate a true passion for senior care, leading by example with BrightStar's Core Values , while striving for excellence and delivering exceptional care to every client.
