Overview:
As a professional care provider at Grand Brook Memory Care, the Director of Health Care (DHC) will be a part of a team that strives to care for and support their residents and their families the same way they would care for their own loved ones. The Director of Health Care will be responsible for the day to day coordination and oversight of all aspects of nursing, restorative and personal care in accordance with current Federal & State regulations as well as local regulations as assigned by the Director of Health Care. The DHC is responsible for planning, facilitating and implementing effective, quality resident care and maintenance of applicable records & reports. Other tasks (not an all inclusive list) include: Develops methods for coordination of nursing services with the assistance to ensure the continuity of the residents' total regimen of care. Monitors and evaluates vital signs, administers medications and performs administrative duties such as medical forms, reports, evaluations, inventory, studies, and charting Maintains professional and effective communication while ensuring the residents feel safe. Promptly responds to all resident care needs and notifies management immediately of changes in conditions, emergency situations and incidents.
Qualifications:
Education:
Completion of a Professional Nursing Program, approved by the state of which you are employed, and licensed to practice as a licensed nurse.
Experience:
Knowledge and experience with residents with Alzheimer's disease and/or related dementia highly desirable.
Other:
Dedication to serve seniors with passion and excellent customer service. Flexibility to perform various duties in service to the residents, the ability to work a flexible schedule including occasional local travel, evenings, weekends, and "on call" hours.
