Full Job Description Responsibilities:
Conducts initial assessment of prospective residents, including off-site visits Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences Coordinates the Community's infection control program while maintaining state regulatory compliance Develop and oversee the Community's medication management program Contributes toward quality assurance and performance improvement initiatives Complies with all rules and regulations regarding confidentiality and privacy of resident information
Qualifications:
Maintains current state nursing licensure. LPN required, RN preferred Minimum of two (2) years nursing supervisory experience Prior senior/supportive living experience strongly preferred Excellent written and verbal communication skills Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members Demonstrates exceptional interpersonal skills and ability to serve with love , compassion , and dignity
