Job Overview Ebenezer is hiring an Assistant Director of Nursing (RN) role who would be responsible for continuity of nursing care by delegating, directing, and monitoring nursing care delivered. Demonstrates management skills through effective problem solving and decision-making, assuring compliance with State and Federal regulations as well as Ebenezer Quality standards. Expert knowledge of Infection prevention information that is shared, audited and sustained at the Ebenezer facilities. Responsibilities This role leads, supervises, and delegates nursing functions on assigned units to ensure care is delivered in accordance with each resident's individualized plan of care. The position ensures staff compliance with state and federal regulations, nursing standards of practice, infection control protocols, and quality assurance standards. Responsibilities include monitoring continuity of care through accurate shift reporting, observing and auditing nursing care and documentation, overseeing mealtime support and diet accuracy, and ensuring medications and treatments are administered according to physician orders and organizational policies. The role also includes evaluating staff performance, conducting reviews and surveys, coordinating team meetings, and providing leadership during emergencies. Additional duties involve assisting with investigations and follow-up related to incidents, medication errors, and vulnerable adult reports; monitoring care delivery throughout the facility; maintaining accurate care plans; participating in interdisciplinary team meetings; communicating changes in care to families and physicians; and serving as a positive role model while keeping leadership informed of concerns and opportunities for improvement. Manage and oversee the facility's infection control program to ensure accurate tracking, reporting, and compliance. Maintains and monitors resident and employee infection data within the PCC system, including infections, illnesses, and immunizations, while collaborating with interdisciplinary teams to ensure data integrity. Partners with the Director of Nursing and Staff Education Nurse to implement Ebenezer policies, procedures, and ongoing education related to infection control and antibiotic stewardship. Supports the implementation of infection control systems and processes, conducts audits and data collection, and leads routine infection control rounds to identify deficiencies and corrective actions, reporting findings to the QAPI committee. Ensures timely reporting of infections and required data to regulatory agencies, including MDH and NHSN, and participates in committees such as Legionella/Water management. Ensures high-quality, safe nursing care by actively participating in meetings, audits, and quality initiatives. Leads and facilitates QAPI efforts, quality improvement projects, and discussions that impact performance metrics such as the 5-star rating. Regularly reviews

reports and readmission cases to identify and drive improvement opportunities, while supporting the implementation of systems and processes that enhance care outcomes. Responsible for preventing and controlling the spread of infection by identifying trends, monitoring for outbreaks, and implementing appropriate care plan interventions. Establishes facility-wide precautions to prevent cross-contamination and collaborates with interdisciplinary teams to promote adherence to infection control practices. Provides ongoing staff education and initiates immediate interventions when concerns or patterns are identified to ensure resident and staff safety. Leads and maintains the facility's Antibiotic Stewardship Program to ensure appropriate antibiotic use and compliance with best practices. Coordinates and directs routine Antibiotic Stewardship meetings, reviews "Timeout" documentation for completeness and appropriate follow-up interventions and provides ongoing education to facility staff and medical providers to promote responsible prescribing and optimal resident outcomes. Represents Nursing Administration by interpreting and communicating Ebenezer policies and procedures to staff, residents, and visitors. Supports the Director of Nursing in planning and implementing departmental meetings, systems, and processes, and fills in for the absence of Director of Nursing, Staff Education and Unit Managers as needed. Serves as an emergency contact and operational lead, participating in management meetings, staffing coordination, hiring, and disciplinary processes in collaboration with the Director of Nursing and Human Resources. Promotes effective communication, professionalism, and compliance, including mandated reporting requirements and scope-of-practice standards. Collaborates with interdisciplinary teams, including Social Services, to support admissions and resident needs, and conducts tours for prospective residents and families. Demonstrates adaptability in managing unexpected situations and participates in designated weekend rotations to ensure continuity of care and operations.
