As an Administrator / Director of Nursing, you will play a critical role in guiding clinical teams, supporting patients and families, and ensuring excellence across all aspects of care. How You'll Make an Impact In this role, you will provide clinical leadership, operational oversight, and team coordination across the hospice program. You will serve as a key liaison across the Interdisciplinary Team (IDT) while ensuring high standards of patient care, compliance, and staff support. Clinical Leadership & Team Coordination Oversee coordination of the Interdisciplinary Team and ensure effective communication across all disciplines Monitor all referrals to ensure timely follow-through and appropriate admissions Assist with patient admission coordination and assignment to appropriate clinical staff Ensure all admission documentation is completed accurately within 24 hours and communicated to the care team Assign Nurse Case Managers to implement and oversee Plans of Care Support case managers with symptom management and clinical decision-making Ensure all changes in patient condition and Plans of Care are communicated appropriately to the team and physicians Staff Supervision, Development & Compliance Supervise and evaluate scheduling and performance of clinical staff, including RNs, LPNs, CNAs, contract staff, and medical directors Recruit, hire, and train clinical staff Conduct annual evaluations and routine supervisory visits for nursing staff and CNAs Provide and coordinate orientation, ongoing education, and in-services related to symptom management and hospice care Participate in ongoing quality improvement efforts to enhance patient care and team performance Ensure adherence to policies, procedures, regulatory requirements, and ethical standards Maintain oversight of documentation practices in collaboration with medical records Quality of Care & Patient Support Participate in IDT meetings addressing admissions, certifications, recertifications, and patient care planning Monitor medication profiles, including formulary and non-formulary utilization, and identify opportunities for improvement Collaborate with medical directors to ensure appropriate and timely patient care Support patients and families in addressing concerns related to services and care Participate in visits and admissions during periods of high census or staffing needs Assist with changes in level of care, including coordination of placements, physician communication, transportation, medications, and DME Operations & Program Development Assist with staffing adjustments and ensure continuity of care Support volunteer and bereavement programs Participate in community outreach, education, and relationship-building to support program growth Contribute to budgetary planning and cost-effective decision-making related to patient care and operations Provide input on policy and procedure development, program growth, and new market expansion Ensure appropriate leadership coverage and operational oversight at all times What You Bring Registered Nurse (RN) licensed and in good standing with the New Hampshire State Board of Nursing Hospice and/or home health experience with a strong clinical background Prior administrative or supervisory experience in hospice required Excellent communication and interpersonal skills, with the ability to engage effectively with staff, physicians, patients, families, and the community Demonstrated ability to lead, motivate, and support clinical teams Strong understanding of hospice philosophy and regulatory requirements Orientation & Professional Development LilyCare is committed to your success as a leader.
