Position Summary Plans, coordinates and provides total nursing care for customers and provides supervision and guidance to clinical team members. Scopes of work may be modified by state specific rules under the Nurse Practice Act. Essential Duties and Responsibilities Initiates and leads team in development of individualized nursing care plans Assesses and documents the customer's condition and nursing needs Accurately and promptly implements physician's orders Assigns nursing care to team members in accordance with the customer's needs and the person's capabilities and qualifications Supervises, directs, and evaluates LN's and CNA's Administers medication, starts IV's and performs treatments for assigned customer and documents treatment as required by PHP, and local, state, and federal rules and regulations Participates in orientation and in-service training Assist physicians with special tests and procedures within the scope of states specified Nursing Practice Act Ensures that supplies are utilized economically and that equipment is clean and maintained in a safe manner Reports and record pertinent observations and reactions regarding customers Coordinates nursing care of customers when scheduled for therapy or procedures by other departments Assists with or institutes emergency measures for sudden adverse developments in customers Interviews applicants and recommends hires. Participates in counseling, disciplinary action and termination of team member(s), to extent permitted by state practice act. Supervises nursing unit, which includes scheduling and directing nursing team members to extent permitted by state practice act Completes semi-annual team member job performance evaluations Required to provide weekly audits as identified Required to adhere to infection control procedures and ensure fellow team members are following guidelines Required to hold daily team member huddles and collect sign-off sheet Audits customer admission to ensure facility is adhering to plan of care; ensure customer medication is accurate and available upon admission Competencies Hosts and/or participates actively in meetings; makes meaningful contributions Takes charge of people/events; assumes leadership in a positive way and treats all team members with fairness and respect Provides clear, objective and timely feedback to employees Creates enthusiasm for team effort; is an effective team builder; sets a model for teamwork that encourages common goals Uses all available and appropriate resources, including team members Interacts frequently with customers; takes initiative to inquire about customer needs and opinions and effectively manages concerns Encourages and develops customer service skills in employees Able to prioritize work. Work Environment This job operates in a post-acute setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Team member is expected to adhere to facility's no-smoking policy. Physical Demands While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 60 pounds. Position Type and Expected Hours of Work This is a position in a post-acute setting that is open 24 hours a day, 365 days a year. Weekend and holiday shifts will be required. Knowledge, Skills, and Abilities Interpersonal skills necessary in order to provide effective leadership of subordinates maintain a variety of internal and external contacts, and work cooperatively with a variety of facilities, managers, executives, and staff Analytical abilities necessary in order to evaluate delivery of care systems and nursing results; respond to deficiencies by identifying and implementing appropriate corrective action; develop and administer sound policies, procedures and budgets; and organize and oversee work of subordinates Skilled in directing and motivating nursing team members Knowledge of training techniques for clinical team members Ability to maintain confidentiality Skilled in work assessment Ability to communicate effectively with internal and external customers at all levels of the organization Proficient computer and Microsoft Application skills. Must have an ability to learn new systems Ability to work with a culturally diverse resident and employee population Must have positive communication skills and demonstrated adequate maturity and patience Proficient in the English language. Excellent communication (verbal and written), customer service and interpersonal skills Must have strong organizational skills with attention to detail, as well as strong leadership skills Must be able to successfully prioritize workload, demonstrate initiative, and analyze situations to make sound decisions based on available data Ability to maintain a calm, professional demeanor under pressure Self-directing with the ability to work with minimal direct supervision Required Education/Experience High School Diploma or Equivalent Graduate of accredited School of Nursing A minimum of five (5) years of progressive nursing experience Required License/Certification Valid State LPN/RN License Valid CPR Certification Preferred Education/Experience College/University Degree in Nursing, Healthcare Management, Business or other related field A minimum of seven (7) years of progressive nursing experience A minimum of three (3) years' experience in a nursing leadership role
