Job Description

Company Overview We are a growing Medicare and Joint Commission-certified home health agency dedicated to delivering compassionate, high-quality care to patients in the comfort of their homes. Our services include Skilled Nursing, Wound Care, Physical Therapy, Occupational Therapy, and Speech Therapy. We are seeking an experienced and motivated Director of Nursing to lead our clinical team and support continued growth and excellence in patient care. Position Summary The Director of Nursing (DON) is responsible for the overall clinical oversight, compliance, and coordination of home health services. This leadership role ensures the delivery of safe, effective, and compliant patient care in accordance with Medicare Conditions of Participation, Joint Commission standards, and state/federal regulations. The ideal candidate is organized, collaborative, clinically strong, and experienced in home health operations, quality assurance, and staff leadership. Responsibilities Provide clinical leadership and oversight for all nursing and therapy services Ensure compliance with Medicare, Joint Commission, and state regulations Supervise and support Skilled Nursing, Wound Care, PT, OT, and ST clinical staff Review clinical documentation for accuracy, compliance, and quality outcomes Oversee patient care coordination and interdisciplinary communication Assist with survey preparedness and regulatory readiness Monitor quality indicators, infection control, and QAPI initiatives Participate in hiring, onboarding, training, and performance management Support clinicians with case management and clinical problem-solving Ensure timely completion of OASIS documentation and plan of care approvals Collaborate with physicians and referral sources to maintain excellent patient care and relationships Participate in on-call rotation as needed Qualifications Active RN (BSN) license in the State of Illinois Minimum of 2 years of home health leadership or supervisory experience preferred Strong knowledge of Medicare Conditions of Participation and Joint Commission standards Experience with OASIS documentation and home health regulations Excellent leadership, communication, and organizational skills Ability to manage multiple priorities in a fast-paced environment Wound care experience preferred Reliable transportation and valid driver's license Details Competitive salary.
