What You'll Do As the Director of Integrated Care, you will serve as our operational and clinical subject matter expert.
Your key responsibilities will include:
Conduct and oversee initial and ongoing client assessments to build highly personalized care plans that focus on maintaining safety, dignity, and independence at home. Oversee care delivery standards across all assigned service areas, ensuring every visit, whether 15 minutes or 24 hours, is delivered with genuine warmth and full attention. Train, mentor, and evaluate caregivers in the "Serve Caregiving Way." You will support their competency development so they can show up prepared, attentive, and steady. Serve as the care compliance expert, ensuring adherence to Texas HCSSA regulations and company policies. You will serve as the administrator or alternate qualified designee on applicable Texas home care licenses. Monitor overall care quality and client satisfaction. Lead incident investigations and coordinate Quality Assessment and Performance Improvement (QAPI) initiatives. Work closely with scheduling, care coordination, and operations teams to ensure seamless care delivery. Provide backup support to Community Operations Managers and participate in on-call leadership coverage as needed. The ideal candidate brings:
Minimum of 5 years of experience in Home Care, Home Health, Assisted Living, or Skilled Nursing. Previous experience as an Administrator, Director of Nursing, Care Manager, Clinical Supervisor, or Compliance Officer is highly preferred. LPN, RN, or Master's in Social Work required. Background in healthcare administration is a plus. A proven track record in conducting client assessments and managing caregiver performance, knowledge of Texas HCSSA regulations and compliance standards a plus A compassionate, client-centered mindset with excellent problem-solving abilities.
