POSITION SUMMARY
The Director of Nursing (DON) is responsible for the overall direction, coordination, and clinical oversight of nursing and therapeutic services provided by HealthSphere Home Care. The DON ensures compliance with Texas HCSSA regulations , CMS Conditions of Participation , and ACHC Home Health Accreditation Standards . The DON provides leadership to clinical staff, ensures high-quality patient care, and maintains continuous survey readiness. 2.
MINIMUM QUALIFICATIONS
(Texas HCSSA Requirements) The DON must meet all of the following: Must be a licensed Registered Nurse (RN) in the State of Texas. Must have at least one year of experience in home health or related health services. Must meet competency requirements for clinical oversight, supervision, and regulatory compliance. Must complete Texas HCSSA Administrator/DON training within required timelines (if applicable). Must be available during posted business hours and during survey activities. 3.
ESSENTIAL DUTIES & RESPONSIBILITIES 3.1
Clinical Oversight Oversees all clinical services, ensuring compliance with CMS CoPs and ACHC standards.
Ensures timely completion of:
Comprehensive assessments (OASIS) Plan of Care (POC) development Physician orders Medication reconciliation Visit documentation Supervises RNs, LVNs, therapists, and HHAs. Conducts supervisory visits per regulatory requirements. 3.2 Patient Care Management Ensures patient needs are met through appropriate service delivery. Reviews clinical documentation for accuracy, completeness, and regulatory compliance. Ensures coordination of care among interdisciplinary team members. Participates in case conferences and clinical reviews. 3.3 Regulatory Compliance Ensures compliance with: Texas

Conditions of Participation (484.55, 484.60, 484.75, 484.80) ACHC Home Health Standards (HH4-HH7) Maintains clinical policies and procedures. Ensures clinical staff competencies are completed and documented. 3.4 Human Resources & Staff Development Participates in hiring, onboarding, and evaluating clinical staff. Ensures personnel files contain required documentation (licenses, TB, background checks, competencies). Provides ongoing education, training, and performance feedback. Ensures staff understand and follow infection control, emergency preparedness, and patient rights policies. 3.5 Quality Management Leads clinical components of the QAPI Program . Reviews clinical indicators, infection control data, and performance improvement projects. Participates in quarterly QAPI meetings and documentation. 3.6 Emergency Preparedness Ensures clinical staff are trained in the Emergency Preparedness Program. Participates in annual training, testing, and after-action reviews. 3.7 Survey Readiness Maintains continuous readiness for ACHC surveys. Prepares clinical records, staff, and documentation for surveyor review. Participates in entrance and exit conferences. Ensures surveyor access to EMR and clinical staff. 4.

Strong knowledge of home health regulations (Texas
HCSSA, CMS
CoPs, ACHC). Clinical leadership and supervisory skills. Ability to manage multiple priorities and deadlines. Strong communication and documentation skills. Proficiency with EMR systems and clinical workflows. Ability to train, mentor, and evaluate clinical staff. 5.
PHYSICAL & WORKING CONDITIONS
Office-based with field supervision as required. Must be available during business hours and for after-hours clinical needs. Must be able to travel within the agency's service area.
