Job Description

Responsibilities:
Provides in-service memory care training at quarterly staff meetings Conducts initial assessment of prospective residents, including off-site visits (home visits, hospital visits) Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members Evaluates and communicates changes in residents' abilities and care status Hires, trains, schedules, and leads the memory care clinical team in providing exceptional care experiences Coordinates with the Activity Director to review and establish a monthly memory care activity calendar Contributes toward quality assurance and performance improvement initiatives Complies with all rules and regulations regarding confidentiality and privacy of resident information
Qualifications:
Maintains current state nursing licensure, RN preferred Minimum of two (2) years nursing supervisory experience preferred Prior senior/supportive living experience required Certified Dementia Practitioner (CDP) per state requirements Excellent written and verbal communication skills Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members Demonstrates exceptional interpersonal skills and ability to serve with love , compassion , and dignity
