Full-time Education/Experience:
RN degree with accredited institution. Current License to practice within the State of Vermont. Extensive experience working with people who are intellectually disabled. Strong communication and advocacy skills. Medical case management skills needed Must demonstrate the ability to: 1. Maintain nursing license in the State of Vermont 2. Function as a senior manager of the organization. Provide effective supervision to Service Coordinators and LPN regarding medical needs to assure for quality of services 3. develop and maintain positive, professional and respectful relationships with individuals served, supervisor, co-workers, community members and other stakeholders. 4. maintain flexibility in work schedule, including but not limited to overnights, weekends, holidays. 5. provide overall medical quality assurance for services provided to individuals served according to State and Federal regulations. 6. provide direct nursing services as needed including but not limited to: nursing evaluation of acute or chronic illness; medication administration, emergency care, schedulingand/or assisting individuals to attend medical appointments. 7. electronically record data within the Agency Electronic Health Record as defined by the person's medical needs in a timely fashion. 8. provide a personal, reliable, inspected vehicle in order to facilitate individual and agency need to include supporting medical appointments and attending home visits. 9. maintain confidentiality practices 10. train, monitor and delegate staff and contractors in medication administration practice
Specific Duties:
1. Maintain respectful and courteous relationship with individual's receiving services, family members, managers, supervisors and co-workers within the agency and the community/employment environments. Facilitate effective team atmosphere, achieve common goalsand represent the culture of the agency 2. Demonstrates sound leadership skills for all teams. As a senior leader within the organization; support, monitor and assure teams are moving in agreed upon direction, resolves conflict and addresses barriers through professional leadership 3. Maintains working knowledge of, adheres to and implements Agency policies/procedures, Division of Developmental Services Procedures, the Divisions Quality Assurance Guidelines including but not limited to the Health and Wellness Guidelines, Positive Behavioral Support Guidelines; and the DD Act of 1996. Maintains working knowledge of; adheres to and implements Agency policies/procedures and Administration Manual practices. 4. Provides and maintains ongoing nursing assessment o f individuals served. Establishes, maintains and coordinates medical data from appropriate sources. Primary medical liaison between physicians, consultants etc. and consumer ISA team regarding medical issues 5. Monitors Agency medical documentation including mediation charts, seizure reports and accident reports and assures for appropriate follow-up. Assures for quality assurance of all medical service documentation. 6. Provides medical support, and training to Service Coordinator regarding individuals serviced. 7. Provides instruction, supervision, delegation and on-going in-service training and to employees, contractors and individuals serviced regarding quality medical practices as outlined in the Agency Policy and Procedure Manual. 8. Conducts Special Care Procedure training for employees, contractors and respite providers according the DD Act and State Health and Wellness guidelines. Assures current documentation of training is maintained in the records. 9. Support and advocate for people served during medical appointments. Support and provide education to people served and their teams during home visits and team meetings. 10. Other duties as assigned
