This position is classified as Exempt and reports to the Director or VP of Therapy. Regional travel of up to 75% is required and expected. Clinical Oversight and Therapy Program Quality Conducting clinical audits and mock surveys to regularly evaluate therapy clinical systems, documentation practices, and service delivery standards across assigned facilities Reviewing therapy program outcomes and recommending evidence-based modifications to ensure residents receive high-quality rehabilitative care Guiding facility therapy teams in performing comprehensive evaluations, developing individualized plans of care with measurable functional goals, and delivering skilled therapeutic interventions Monitoring therapy department performance and adjusting recommendations based on clinical presentation, objective outcomes, and regulatory requirements Ensuring discharge planning, transition-of-care coordination, and community reintegration recommendations reflect current best practices Providing expert consultation on the use of assistive devices, orthotics, prosthetics, and adaptive equipment training for patients and caregivers Serving as a senior clinical resource to therapy staff, interdisciplinary team members, patients, families, and facility leadership across the region Regulatory Compliance and Survey Readiness Communicating best practices across Physical Therapy, Occupational Therapy, and Speech Language Pathology disciplines and recommending relevant clinical changes in a timely manner Assisting in the development of therapy clinical policies, procedures, and training programs to ensure quality and regulatory consistency across assigned facilities Assisting in the timely and accurate completion of Plans of Correction and other documentation required by government or regulatory agencies Documentation, Reporting, and Analytics Analyzing costs, revenues, therapy utilization data, and performance measures to evaluate results, recommend goals, and monitor regional progress Ensuring therapy documentation across assigned facilities is timely, accurate, and compliant with federal, state, payer, and organizational requirements Collaborating with cross-functional business partners to ensure organizational alignment on therapy clinical measures Making written and oral reports and recommendations to Administrators and the Director or VP of Clinical as required Supervision, Staff Development, and Mentorship Providing feedback to facility leadership on Therapy Directors and clinical therapy personnel with respect to hiring, training, performance management, coaching, and separation as needed Supervising and evaluating the clinical practice of therapy department leaders and personnel in accordance with California state law, professional regulations, and organizational standards Mentoring and coaching therapy staff on clinical best practices, documentation standards, evidence-based interventions, and quality of care expectations Interdisciplinary Collaboration and Care Planning Participating in care conferences, quality meetings, and collaborative planning initiatives across assigned facilities Guiding therapy teams in the development, implementation, and quarterly review of resident assessments (MDS) and individualized care plans Communicating therapy evaluation findings, progress updates, and discharge outcomes to physicians, interdisciplinary team members, residents, and families What You Bring Current, active, and unrestricted clinical therapy license in the State of California in Physical Therapy, Occupational Therapy, or Speech Language Pathology — required Graduate of an accredited therapy program (Bachelor's, Master's, or Doctorate as applicable to discipline) — required Minimum three to five years of progressive clinical experience in a post-acute, skilled nursing, or hospital rehabilitation setting — required Five or more years as a Therapy Department Director or equivalent multi-site clinical leadership role — strongly preferred • Demonstrated knowledge of MDS, PDPM
reimbursement methodology, and skilled rehabilitation regulatory requirements Current CPR/BLS certification Ongoing compliance with continuing education requirements established by the applicable California state therapy licensing board Ability to travel up to 75% of the time across assigned Northern California facilities and region Strong verbal and written communication skills with the ability to exercise sound professional judgment Demonstrated ability to lead and influence outcomes without direct authority across multiple facility environments Strong organizational skills and the ability to manage multiple concurrent priorities across a complex regional portfolio Physical Requirements The Regional Therapy Resource must be able to move intermittently throughout the workday across multiple facility environments, including prolonged standing, walking, bending, reaching, kneeling, and stair navigation during facility rounds and clinical oversight activities. Patient transfer and mobility assistance may be required during clinical consultation visits. Significant regional travel by automobile, train, or airplane is required and expected.
