Job Description

Job Summary and Qualifications What you will do in this role: Act as a liaison between internal and external referral sources and rehab operations. Be responsible for establishing relationships with case managers, physicians, payors, and other referral sources. Ensure appropriate patient placement through clinical assessments and screening in collaboration with the Rehab Medical Director. Act as the responsible party focused on the referral and admission process of the rehab program. Will be required to pass facility credentialing for Functional Independence Measurement (FIM) within 90 days upon hire, then maintain current certification.
What qualifications you will need:
Basic Cardiac Life Support must be obtained within 30 days of employment start date Associate degree (RN) Registered Nurse, or (COTA) Certified Occupational Therapy Assistant, or (OTR) Occupational Therapist Registered, or (LPT) Licensed Physical Therapist, or (LPTA) Licensed Physical Therapist Assistant, or (SLP) Speech-Language Pathologist • No Travel Required • 2 years experience Required Years of Experience Benefits Centerpoint Medical Center, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
