Assistant Director of Subacute Services Garden Gate Rehabilitation & Nursing Cheektowaga, NY Salary:
Up to $81,120 annually ($36.00-$39.00/hour equivalent) Position Summary Garden Gate Rehabilitation & Nursing is seeking an Assistant Director of Subacute Services to help lead and grow our short-term rehabilitation and post-acute care program. This operational leadership role serves as a key liaison between clinical, admissions, therapy, case management, social work, and nursing teams to ensure exceptional outcomes for residents receiving short-term rehabilitation services. The ideal candidate is a highly organized, relationship-driven leader who understands the unique needs of post-acute care and rehabilitation. Candidates with experience in skilled nursing, rehabilitation operations, healthcare administration, admissions, case management, MDS, managed care, or reimbursement are encouraged to apply. An active nursing license is welcomed but not required. Key Responsibilities Subacute Program Leadership Support daily operations of the subacute rehabilitation program. Assist in developing and maintaining a high-performing post-acute care service line. Monitor program performance, resident outcomes, and operational goals. Promote a positive rehabilitation experience for residents and families. Census & Growth Collaborate with admissions, hospital liaisons, and referral sources to support subacute census growth. Assist with evaluating referrals and coordinating successful admissions. Participate in facility marketing and relationship-building efforts with healthcare partners. Case Mix & Reimbursement Support Maintain working knowledge of Medicare, managed care, PDPM, and rehabilitation reimbursement methodologies. Monitor case mix trends and identify opportunities for reimbursement optimization. Collaborate with MDS, therapy, and interdisciplinary teams to ensure accurate documentation and resident classification. Support initiatives that improve quality outcomes while maximizing appropriate reimbursement. Interdisciplinary Coordination Facilitate communication between nursing, therapy, social services, admissions, and providers. Assist with care transition planning and discharge readiness. Participate in care conferences and interdisciplinary meetings. Help remove barriers that impact resident progress and length of stay. Quality & Customer Experience Monitor resident satisfaction and family feedback. Identify opportunities to improve the rehabilitation experience. Support quality improvement initiatives focused on outcomes, readmissions, and resident satisfaction. Represent the organization professionally with residents, families, referral partners, and community stakeholders. Leadership & Team Development Serve as a visible leader and resource for staff. Promote accountability, teamwork, and resident-centered care. Assist department leaders with operational initiatives and special projects. Support facility leadership in achieving strategic goals for the subacute program. Qualifications Required Associate degree or equivalent healthcare experience. Minimum of 3 years of experience in healthcare, rehabilitation, skilled nursing, post-acute care, admissions, case management, MDS, managed care, or related field. Strong organizational, communication, and leadership skills. Ability to analyze operational data and drive performance improvements. Preferred Bachelor's degree in Healthcare Administration, Business Administration, Rehabilitation Services, Nursing, or related field. Knowledge of Medicare, managed care, PDPM, rehabilitation reimbursement, and case mix management preferred. Skilled nursing facility or rehabilitation experience.
