  • Skilled Nursing Facility Job Overview We are seeking a dynamic and experienced Director of Rehabilitation to lead and oversee rehabilitation services within our Skilled Nursing Facility.
This leadership role is responsible for ensuring the delivery of high-quality physical, occupational, and speech therapy services that support residents in achieving their highest level of function, independence, and quality of life. The Director of Rehabilitation will provide clinical and operational oversight, promote interdisciplinary collaboration, ensure regulatory compliance, and drive positive resident outcomes while maintaining efficient department operations. This is an excellent opportunity for a motivated rehabilitation leader who is passionate about resident-centered care, team development, and excellence in post-acute and long-term care services. Responsibilities Direct the daily operations of the rehabilitation department, including physical, occupational, and speech therapy services. Develop, implement, and monitor rehabilitation programs that support residents' functional goals, discharge planning, and overall quality of life. Lead, mentor, and support a multidisciplinary team of therapists and rehabilitation staff to promote clinical excellence, professional growth, and employee engagement. Ensure compliance with all federal, state, and local regulations, including Medicare Part A and Part B requirements, documentation standards, and facility policies. Collaborate closely with nursing, physicians, social services, case management, and facility leadership to coordinate resident care and optimize outcomes. Participate in care plan meetings, quality assurance initiatives, and interdisciplinary team meetings to support resident-centered care. Monitor key performance indicators, including clinical outcomes, therapy utilization, resident satisfaction, length of stay, rehospitalization rates, and discharge success. Manage departmental budgets, productivity expectations, staffing schedules, and equipment needs to ensure operational efficiency and fiscal responsibility. Support census development efforts by collaborating with admissions and referral sources to ensure appropriate rehabilitation service delivery. Maintain current knowledge of skilled nursing, post-acute care, and rehabilitation best practices to drive continuous quality improvement. Qualifications & Experience Licensed Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) in good standing, as required by state regulations. Strong knowledge of Medicare, Medicaid, PDPM, therapy documentation requirements, and skilled nursing regulations. Demonstrated success managing multidisciplinary rehabilitation teams and achieving positive clinical and operational outcomes. Excellent leadership, communication, organizational, and problem-solving skills. Experience with rehabilitation management software, electronic medical records, and productivity management systems preferred.
