Job Description

PRN Home Health & Therapy PRN Home Health & Therapy is seeking an experienced and forward‑thinking Director of Physical Therapy to lead our therapy services, champion clinical excellence, and support high‑quality patient care across our home health programs. This leadership role blends strategic oversight, hands‑on clinical practice, interdisciplinary collaboration, and program development to advance our mission as a trusted provider in the communities we serve. Key Responsibilities Leadership & Program Management Provide strategic direction for therapy services to support organizational growth and overall company success Oversee, evaluate, and enhance therapy programs to ensure high‑quality, patient‑centered care Lead program development initiatives that improve patient outcomes and expand service offerings Ensure compliance with agency policies, professional standards, state and federal regulations, and accreditation requirements Support ongoing certification and accreditation efforts through quality assurance and regulatory readiness Represent PRN Home Health & Therapy professionally within the community and industry Interdisciplinary Collaboration Participate in community resource meetings, advisory committees, marketing initiatives, department meetings, and interdisciplinary care conferences Serve as a clinical resource for team members across all disciplines Coordinate care with physicians, nurses, social workers, occupational therapists, PTAs, home health aides, hospice teams, and administrative staff to ensure seamless patient care Clinical Responsibilities Conduct comprehensive assessments and deliver evidence‑based physical therapy interventions in the home setting Manage patient caseloads, ensuring timely documentation, follow‑up, and continuity of care Supervise and mentor physical therapy assistants, ensuring adherence to care plans and professional standards Provide education on fall‑risk reduction, injury prevention, and safe mobility practices Promote health, wellness, and fitness initiatives tailored to the geriatric population Professional Development & Organizational Support Maintain and expand clinical and managerial expertise through continuing education and professional development Establish service priorities that align with patient needs and organizational goals Contribute to organizational initiatives, quality improvement efforts, and strategic planning Qualifications Licensed Physical Therapist (PT) or other qualified therapy professional Experience in home health, hospice, or geriatric care preferred Demonstrated leadership, communication, and organizational skills Ability to collaborate effectively within an interdisciplinary team Knowledge of regulatory standards and accreditation processes
