Seven Acres is home to one of the largest long-term care facilities in the Houston area, caring for those who require monitoring and support, as well as those who need significant long-term clinical intervention. Seven Acres' comprehensive professional support enhances coordinated care and allows residents to see specialists in the comfort of their homes. Seven Acres provides full-time medical services, including on-site physicians, through a contract with Trumen Physicians & Associates PLLC. Seven Acres is one of the largest long-term care facilities in the Houston area, caring for individuals who require 24/7 monitoring and support, as well as those who need significant long-term clinical intervention. Essential Duties and Responsibilities to Assist with: Supervise, orient, and train Rehab Department Treat/ Direct Patient Care Coordinate treatment activity between disciplines and team members Assist with the interview process for hiring Ensure that therapists and therapy assistants are compliant with documentation. Communicate effectively with administration regarding department programs, goals, objectives, risks, and successes. Ensure timely and accurate billing of services provided and completion of required documentation and maintain quality of services. •Attends all mandatory meetings and training Assures all patient care activities are performed according to the company policies and procedures Contribute to client relations and community education activities
Qualifications:
Current and active state licensure as a PT, OT, SLP, OTA or PTA. Minimum of 1 year of clinical experience; prior leadership or supervisory experience is strongly preferred. Knowledge of therapy regulations, reimbursement, and documentation standards (Medicare/Medicaid, SNF PPS, PDPM, etc.). Strong organizational, communication, and interpersonal skills. Ability to lead a multidisciplinary team with professionalism and accountability. CPR certification and other credentials as required by state/facility.
Preferred Qualifications:
Experience in skilled nursing, home health, or inpatient rehab settings. Familiarity with EMR systems and therapy management software. Demonstrated leadership in quality improvement initiatives or program development.
