Job Description

Our care philosophy is guided by the following core principles: Dignity and Respect Information Sharing Participation Collaboration The Liaison Clinician is responsible for performing assigned duties in accordance with NRHN policies, procedures, and standards of care while supporting the organization's mission, values, and commitment to quality outcomes. Essential Responsibilities The Liaison Clinician will thoroughly and accurately complete pre-admission screenings, utilizing critical judgment in accordance with established field acceptance criteria. The Liaison Clinician serves as the hospital's representative to patients, referring institutions and agencies, physicians, and family members. Qualifications Registered Nurse, Licensed Practical Nurse, Social Worker or Therapist (physical, occupational, respiratory) with current licensure in New Hampshire or Massachusetts and appropriate experience in clinical evaluation and assessment of rehab patients will be considered. Three years of acute medical surgical experience. Critical care, rehabilitation, insurance and marketing experience preferred. Valid driver's license and safe driving record as defined by Hospital Insurance company. Able to multi-task with strong interpersonal skills. Strong Computer skills required, and knowledge of electronic products preferred.
