Every day you will establish and update individualized exercise prescriptions, orient patients to equipment and safety protocols, and conduct functional fitness testing and body composition analysis. You will provide essential patient and family education, supervise and monitor exercise sessions, meticulously document and maintain current clinical data and program records, adjusting treatment plans as appropriate. To be successful in this role, you will demonstrate a deep understanding of patient needs, consistently working collaboratively in a diverse and multicultural environment. You will effectively partner with physicians and clinical staff in goal-setting, consistently utilizing work practices aligned with safety, security, and infection control policies, all while actively contributing to a positive quality work environment. Collaborates with physicians and nursing staff to identify patients requiring mobilization plans of care Collaborates with nursing to develop and implement individualized mobilization plans of care for patients Collaborates with nursing to evaluate effectiveness of the plan of care in achieving desired outcomes and revises plan of care as needed Interdepartmental liaison between the recovery room, intensive care areas and the floors to promote continuity of care and discharge planning Communicates treatment plan with collaborative team members
