Program Manager Pay Range:
$71,000 to $88,000 per annual Summary Under the supervision of the Division Director and /or Program Manager, the Mental Health Forensic Therapist/ Care Coordinator II is responsible for but not limited to planning, facilitating, implementing, and coordinating mental health / case management /crisis intervention services to chronically mentally ill adults with history of non-compliance with treatment including substance abuse, homelessness, and/or incarceration. Essential Functions
  • Utilizes Evidence-based mental health practices.
  • Provides mental health, case management (housing/ benefits assistance/vocational/socialization & recreation), substance use services and other community resources, providing linkage, and referrals to ensure continuous system of care.
  • Provides Crisis residential treatment as a positive, temporary alternative for people experiencing an acute psychiatric episode or intense emotional distress who might otherwise face voluntary or involuntary commitment.
  • Coordinates services with various public and private community health, mental health, and social service agencies in treatment of clients.
  • Outreach and engagement.
  • Reports program activities and development, crises, and any other pertinent information to appropriate chain of command.
  • Transport client(s) to connect with necessary community resources.
  • Establish and maintain a therapeutic relationship with client and family members.
  • Participation in the treatment recommendations.
  • Provide and/or assist with on-call emergency coverage.
  • Provide daily/weekly contact with client to ensure continuous system of care.
  • Participate, coordinate and implement cooperative arrangements and/or team conferences with various inter-disciplinary team members in the development and implementation of client's treatment plan and aftercare plan (including Client Care Committee meeting).
  • Maintain and submit billing records in a timely manner.
  • Maintain and update client's files and documentation in accordance with DHS-Office and Diversion Re-entry guidelines.
  • Maintain productivity rate as required.
  • Perform other tasks and responsibilities as assigned by the Division Director (AP Residential Treatment) and/or Program Manager. Secondary Functions
  • Provide consultation to agencies and constituents of the community as appropriate.
  • Other duties as needed Minimum Qualifications
  • Master in Social Work or MFT from an accredited college or university.
  • Licensed or Registered with California BBS.
  • Verification of Employment Eligibility and background check.
  • Must have reliable transportation, valid California Driver License, and proof of automobile insurance coverage. - Minimum of 1 year experience providing crisis interventions with individuals exhibiting acute mental health symptoms.
  • Basic Computer software programs (e.g. Word processing, spreadsheet and internet).
  • Knowledge and ability to relate to people with mental illness.
  • Knowledge of DSM-V and Mental Illness.
  • Good writing and oral (communication) skills are necessary, strong interpersonal and social skills and time management skills necessary
  • Ability to develop and maintain effective working relations with program and with community representatives and contractors.
  • Ability to work both independently and as a member of a team. Non-Essential Qualifications
  • Knowledge of community resources
  • On-call, Overtime, holiday or weekend work maybe required
  • Preferred one year of direct service experience in a hospital or mental health setting which has an interdisciplinary team approach.
  • Strong personality to deal with chronically mentally ill client.
Supervisory Responsibilities N/A Environmental Conditions The incumbent may be exposed to weather conditions and noise level prevalent at the time, unfavorable driving/parking conditions, and aggressive clients. Physical Requirements Typically spends time sitting, standing, walking, driving, carrying (max. 20 lbs. or less), listening, speaking. Mental Requirements The incumbent must be able to handle any/all of the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules; be able to process information, think, and conceptualize.
