Job Description

CURRENT HOLLAND HOSPITAL EMPLOYEES

This position is responsible for the assessment and treatment of patients on the inpatient psychiatric unit. The therapist will complete an initial evaluation, gather input from the patient's significant others, and collaborate with the multidisciplinary team to develop a person centered treatment plan. Therapeutic interventions include individual therapy, family sessions, and group therapy. Case management responsibilities focus on awareness of and appropriate utilization of financial resources, planning aftercare, and assisting the family to locate appropriate placement if needed. The therapist will maintain current independent licensure and credentials, will participate in continuing education, and will comply with unit specific competency requirements.

USD Hourly Employment Type:
Part Time, 48 hours every 2 weeks.
Schedule:
Day shift, 7:30am to 4pm, One weekend a month required. 1 holiday per year required.
Requirements:
Masters Degree in Social Work, Counseling or Psychology required. Current Michigan certification required. Current BLS for the Healthcare Provider Certification completed by 60 days after hire required.
PATIENT CARE
  • ASSESSMENT Completes timely and accurate initial assessment, including patient's psychosocial history, previous psychiatric illness, family relationships, stressors, and support systems.
Gathers pertinent data from a patient's family or significant others. Demonstrates ability to continually assess patient's status and progress toward goals. Is alert to safety concerns and is able to recommend appropriate levels of intervention for the patient's protection.
PATIENT CARE
  • PLANNING Includes patient, family or significant others, and members of the multidisciplinary treatment team in the development of a patient centered care plan.
Identifies specific, measurable goals and the appropriate therapeutic interventions to achieve them. Prepares a written treatment plan, presents it to the patient. Obtains the patient's signature indicating understanding and acceptance of the plan and provides the patient a copy of the document. Collaborates with the interdisciplinary treatment team to evaluate patient progress and update the written plan of care.
PATIENT CARE
  • IMPLEMENTATION Provides individual, family, and group therapy when indicated.
Functions as a case manger by appropriate use of financial resources, participation in utilization management, collaboration with outpatient providers, and coordination of discharge plans including aftercare arrangements. Works collaboratively with family to resolve placement issues if needed . Is knowledgeable regarding community resources and makes appropriate referrals.
PATIENT CARE
  • DOCUMENTATION Makes accurate and timely entries in the patient's electronic chart to record interactions with patient or family, referral source collaboration, or discharge planning.
Appropriately completes necessary forms
FUNCTIONS AS PROFESSIONAL MEMBER OF THE MULTIDISCIPLINARY TREATMENT TEAM
Attends and participates in team meetings. Works collaboratively with team members, seeking and incorporating their input. Promotes interpersonal team relationships by giving/receiving open and direct feedback among team members. Maintains professional licensure through accumulation of required CEU's. Attends staff education meetings and meets mandatory unit competencies.
  • Independent Practice Certification in area of expertise (LMSW, Substance Abuse, CAC, LPC).
  • Computer literate required
